Description

The Blended Case Manager will partake in the community based program in the Altoona, PA area, working along side of 11 Blended Case Managers.  80% of the day is spent in the community, serving both adult and child populations. The max caseload for one BCM is 30 individuals.

 

Dress code is business casual. The schedule is M-F 8a-4p, but on call availability a must for emergent situations.

Responsibilities:

  • Provide case management services which will assist individuals in attaining the following outcomes: Reduction in inpatient days. Maintain compliance with mental health treatment appointments. Minimum contacts with Crisis Center. Minimum of inappropriate behavioral episodes requiring police or Case Manager intervention. Entrance into appropriate vocational or educational services.
  • Endeavor to achieve and maintain quality and consumer satisfaction in the delivery of services to internal/external customers.
  • Demonstrates competency in welcoming expectations.
  • Represent and participate in Agency professional and community activities as requested.
  • Maintain and protect the individual and Agency confidentiality.
  • Will provide case management services, using a system of recovery that integrates mental health services, drug and alcohol services and community resources, to an assigned caseload up to 30 individuals. Identify the psychiatric, social, emotional and economic needs of individuals through a continuous process in conjunction with the individual, involved family members and supervisor. Emphasis during assessment shall be placed on client precipitants to hospitalization, strengths, interests, and personal goals. Complete environmental matrix for all individuals to determine level of case management service required. Establish recovery oriented service plans for individuals in cooperation with the individual, supervisor and other professionals. Assist individuals in writing Safety Crisis Plans. Implement service plans in cooperation with the individual, family and other professionals.
  • Demonstrate support of Agency mission and philosophy in delivery of services.
  • Promote personal and co-worker safety during work duties.
  • Participate in educational programs, quality assurance, and quality improvement activities.
  • Evaluate the individual's progress based upon the service plans at required times and as special needs arise. Revise service plans in cooperation with the individual and supervisor. Refer individuals to allied community organizations and agencies for assistance with needs that cannot be met by the program or Agency. Advocate for individuals in securing necessary services and resources to meet their needs. Communicate effectively regarding individual progress through reports, records, and team conferences. Assist individuals and families managing personal crisis.
  • Function in the Case Manager role utilizing work time under the following guidelines: At least 75% of the time is to be spent out of the office involved with individuals or advocacy for individuals. No more than 25% of time is to be spent with administrative meetings and paperwork. Maintain productivity standard as established by supervisor annually. Complete all paperwork required by regulations and Home Nursing Agency accurately, legibly and in the allowable time frame. Complete and forward all required paperwork to CBHNP in the allowable time frame. Contact CBHNP care managers when needed for the provision of case management services or on behalf of other behavioral health services. Will be available 24-hour/7 days per week on an on call basis for assigned caseload in situations involving crisis intervention services. This may include face-to-face involvement when involuntary hospitalization is being considered or other appropriate situations.

 

Qualifications

  • Bachelor's Degree in Sociology, Social Work, Psychology, Criminology or related field required. 
  • Minimum of one year related work experience required. 
  • Competency in computer skills in a Windows based environment.
  • Access to transportation in the Agency service area with valid Pennsylvania driver's license and auto liability insurance. 
  • Availability for evening and weekend on-call basis.
