UPMC Home Health and Hospice in Erie is hiring for a Full Time Home Care Aide!
This is a Full Time position Monday-Friday 8A-430P. This position requires Rotating Weekends and Holidays.

Responsibilities:
- Identify, develop and participate in process improvement opportunities within the home health agency that will enhance the quality of services we provide.
- Fulfill agency responsibilities by completing all required forms accurately and in a timely manner.
- Communicate appropriately with the Interdisciplinary health team in a timely manner, reporting any changes in patient's condition to appropriate professional staff.
- Demonstrates understanding of hospice philosophy of care.
- Under the supervision of a registered nurse, provides services to patients in home health and private duty as needed.
- Provide safe and direct personal care to patients under direct supervision of appropriate professional by performing a variety of assignments as defined in the Home Care Aide Competency Evaluation Program.
- Participate in all required agency meetings.
- Provides physical and emotional support to the patient and family during the dying process.
Qualifications
- High School Diploma or GED - Required
- Must have successfully completed a state-established or other training program that meets the requirements of 484.36(a) and a competency evaluation program or state licensure program that meets the requirements 484.36(b) or (e), or a competency evaluation program or state licensure program that meets the requirements of 484.36(b) or (e) - Required
- One (1) year experience - Required
- Completes hospice orientation addressing the needs and concerns of patients and families coping with terminal illness
Licensure, Certifications, and Clearances:
Registration with Nurse Aide Registry Preferred CPR required based on AHA standards that include both a didactic and skills demonstration component within 30 days of hire
- Automotive Insurance
- Basic Life Support (BLS) OR Cardiopulmonary Resuscitation (CPR)
- Driver's License
