UPMC Community Provider Services, Community Based CRNP 

is hiring several Certified Registered Nurse Practitioners to support our Provider Services. The schedule will offer Mondays-Fridays, plus 24/7 call (rotating).  The CRNP will work from home with some in-person meeting requirements and in-home visits.  The CRNP needs to have excellent communication skills over the phone, be comfortable with Telehealth encounters, some in-home visit requirement. Productivity is at 5 visits per day, to include a combination of Medical Wellness visits, Health Plan palliative care visits (both in-home and virtual). Provide high level medical care to complex patients with the goal to prevent hospitalization and provide advance care planning. One year or more of experience is preferred.

Purpose:
The Certified Registered Nurse Practitioner (CRNP) works independently and in collaboration with physicians to provide consultation to patients living with advanced chronic illness, or potentially life-limiting conditions. They assist with the diagnosis, treatment and management of acute and chronic conditions by conducting a comprehensive assessment of symptoms and development of patient centered goals of care. . The CRNP is an integral part of the clinical management team and works collaboratively with the interdisciplinary team to manage the clinical and medical needs of the patients. The CRNP communicates proactively with attending physicians regarding the clinical condition of their patients. The CRNP also functions as a liaison between nurses, physicians and patients.  The position will require travel in the Pittsburgh and surrounding areas. Must have a valid driver's license in good standing and reliable transportation.

Responsibilities:

  • Communicate daily with interdisciplinary team to identify patients with clinical or medical urgent needs.
  • Provide clinical recommendations to nursing staff to address the medical needs of the patients
  • Perform medical evaluation of patients in response to identified need/request from nursing, attending physician or other staff
  • Conduct comprehensive, focused assessments of patients and families with palliative care needs.
  • Document findings in patient's medical record
  • Order, perform, and interpret laboratory and radiology tests within scope of professional practice.
  • Communicates with UPMC Health Plan care management for Health Plan members regarding patient care plan, Advance Care Plans, and discharge planning where appropriate
  • Collaborate with nursing staff and management to instruct and promote assessment skills of nursing staff
  • Participate in facility Quality Improvement process through meeting attendance, audit of outcome measures and input regarding processes and procedures
  • Communicates effectively with patients and families for understanding of diagnosis, prognosis and treatment plans
  • Participate in recommendations for improved medical care management, ordering medications, laboratory test, diagnostic studies, consults, etc. when appropriate
  • Engages with geriatric care management designated for Health Plan members, to facilitate discharge as needed
  • Work with patients and families to determine their understanding of the patient's condition, proposed treatment plan and treatment options.
  • Provide onsite medical management in collaboration with Medical Director and Attending Physicians.
  • Formulate, implement, monitor, evaluate and modify palliative care plans with input from palliative care team and other collaborating care providers.
  • Initiate referrals and consultations with other licensed professional health care providers, and consult with other licensed professional health care providers as indicated
  • Assist in all facets of care coordination for palliative medicine referrals.
  • Precept School of Nursing Nurse Practitioner students
  • Participates as a member of the facility's Interdisciplinary Palliative Care team. Supports creation of care plans that reflect the values and goals of patients and families. Supports Advance Care Planning as indicated. Ensures adherence of clinical care to established care plans through ongoing communication with patients and their families / significant others, and with attending physicians
  • Participate and lead (as appropriate) patient and family meetings
  • Assist with diagnosis, treatment and management of acute and chronic health conditions
  • Interact with the patient and family/significant other providing information and support to develop and implement treatment plan addressing physical, emotional, psychosocial and spiritual support.
  • Evaluate effectiveness of the treatment plan and make recommendations for changes when indicated
  • Provide palliative education to patients and families regarding chronic disease management, symptom management and quality of life expectations.


Qualifications

Graduate of an approved Nurse Practitioner program is required.

Licensure, Certifications, and Clearances:
National certification, eligible for licensure by Pennsylvania board of nursing, eligible for certification as a CRNP by Pennsylvania state board of nursingCPR required based on AHA standards that include both a didactic and skills demonstration component within 30 days of hire

  • Basic Life Support (BLS) OR Cardiopulmonary Resuscitation (CPR)
