Description
UPMC Health Plan is hiring a part-time Community Paramedic to support the Connect Community Paramedics department. This is a daylight role (work days typically begin at 7:00 a.m. or 8:00 a.m.) and will be based out of the Pittsburgh (Northside - Harbor Gardens), PA office location. Some travel will be required.
A minimum of two 8-hour shifts per week is preferred.
The Community Paramedic is responsible for staffing the daily operations of the CONNECT- Community Paramedic Program. The Community Paramedic is responsible for caring for vulnerable patients in their home or other community setting to provide health promotion, disease management, care transitions and other services as needed. Position is grant-funded and will only last as long as funding remains available for the program.
Responsibilities:
- Interact with EMS personnel, ED staff and Care Management teams as necessary to coordinate care for patients.
- Contact and help patients enroll in various social service programs as necessary to help patients close gaps in care.
- Document and report all findings as requested.
- Schedule and visit patients to complete social ecological assessments and follow up of patients as assigned.
- Complete all training required in the timeframes established.
Qualifications
- Highschool diploma or equivalent
- Minimum of 1-year experience in public health, EMS, health screenings or other health related field
- Proficient in MS Word/Excel
- Ability to interact effectively with senior management, EMTs/paramedics, and program participants
- Reliable transportation
- Ability to work early morning hours and weekend hours as necessary.
- Ability to work at least one 8-12 hour shift per week.
- Prefer candidates currently working or living in CONNECT communities and/or Allegheny County.
Licensure, Certifications, and Clearances:
Active or previous EMT or Paramedic Certification
- Automotive Insurance
- Driver's License
- Act 34 with renewal
