Purpose:
UPMC Community Medicine is currently hiring a Social Work Case Manager to coordinate and facilitate patient-centered care to patients with Substance Use Disorders that are receiving Medication Assisted Treatment (MAT) at the UPMC Shadyside Family Health Center. The UPMC Shadyside Family Health Center is a Family Medicine staffed primary care center with multi-disciplinary team services including physicians, behavioral health, and case management services.
This is a Part-Time position with shifts ranging Monday - Friday during daylight hours.
The Social Work Case Manager will provide family health center patients with support and assistance in accessing resources and programs. Assist in improving the overall physical and mental health of patients by coordinating care. Collaborating with behavioral health and medical providers to identify complex cases.
This position is grant funded.
Responsibilities:- Provide Social Work services, including conducting bio-psycho-social assessments, crisis interventions and short or long term treatment.
- Work one-on-one with patients to address their mental health needs and be an integral member of a care team that includes a primary care physician, psychiatrist, nurses, pharmacists and community partners.
- Collaborate to develop a care plan for each patient that will address their unique needs.
- Participate in MAT case conferences, and other team meetings as assigned, as well as assist with managing MAT patient registries to promote patient care adherence.
- Evaluate and initiate appropriate referrals for assistance through County, State, and Federal programs.
- Assisting patients with connections to community resources in the life domains: (1) housing (2) legal (3) food (4) utilities or (5) health care services
- Assist patients in completing application for free medication programs.
- Provide information and education about health insurance, medical assistance, Medicare, and UPMC financial assistance.
- Complete social work consults with health center patients from birth to geriatric.
- Case management of health center patients to ensure that connection with the correct provider and follow up.
- Working with patients to complete necessary forms
Qualifications
- Master's degree in social work
- 1-2 years' experience working or volunteering in the human services sector

Knowledge or interest in addiction medicine and substance abuse treatment preferred
Licensure, Certifications, and Clearances:
- LCSW strongly preferred
- LSW preferred
