The RESIDENT SUPPORT COORDINATOR is responsible for the co-ordination and management of Resident Support Plans, Staff education and Medications Systems management. In this capacity, the RSC completes and maintains the resident assessments and support plans according to required scheduling (DPW); coordinates and maintains records of employee training including annual Mandatory In Services, orientation, competency evaluation and coordination of external education providers; and coordinates and maintains the supporting systems for medication administration ,which may include ordering of medications, management and review of MARs and follow-up for order/refill as needed and indicated.
Responsibilities:
- Track levels of care for residents in facility making recommendations while coordinating changes, as necessary, with Director of Resident Care/Charge Person and communicating changes with staff as necessary.
- Coordinate training and orientation program for employees.
- Complete Personal Care Home Assessment, Facility Assessment Checklist and Support Plan of residents on initial admissions and annually.
- Review support plan with all nursing shifts in order to guarantee that support plan accurately reflects resident's needs on a 24-hour basis.
- Coordinate the review and update of monthly MAR's
- Maintain process for coordination and record keeping for employee CPR course completion.
- Assists DRC with scheduling of staff
- Track annual assessments and support plan to coordinate with annual DPW approved Medical Evaluation.
- Schedules and completes support plans

- Schedule physician appointment to obtain DPW approved Medical Evaluation.
- Update resident support plans as needed.
- Provide direct training and coordination of training for direct care staff. Assure that all training requirements (DPW & UPMC SRC) are included in employee's orientation and annual training plan.
- Completes increased volume of support plans
- Maintain records of employee training.
- Communicate with resident's families as needed to coordinate resident care.
- Assist DRC with the auditing of resident's charts, MARs, Narcotics sheets and ADLs.
- Monitor and track resident's medication /prescription/refill needs and manage process to obtain refills and re-orders as needed.
Qualifications
High school diploma required
Minimum two (2) years experience as a nursing assistant required
Minimum of one (1) year experience working with the elderly / LTC environment required
Knowledge of medical terminology required
Strong computer skills preferred
Licensure, Certifications, and Clearances:
CPR required based on AHA standards that include both a didactic and skills demonstration component within 30 days of hireCertified as CPR instructor within 6 months of hireRequires LPN licensure to support DRC with additional nursing duties
- Basic Life Support (BLS) OR Cardiopulmonary Resuscitation (CPR) OR First Aid Certification
