Description

 

Somerset Health Services is hiring a Part Time SCA Case Manager to support the Somerset Single County Authority for Drug & Alcohol office in Somerset, PA!

The shift of this position is 9:00 a.m. to 3:00 p.m. approximately 3 days per week.

 

Purpose:
This is professional work in the field of drug and alcohol providing a full range of case management services.

Responsibilities:

  • Represent the SCA by attending numerous meetings and sitting on appointed boards as needed.
  • Proficient Computer Skills   
    • Create and Maintain accurate SCA reports as required. 
    • Data Entry and/or navigation in/of state approved systems.
  • Customer Service
    • Adheres to the Customer Service Standards and Behaviors as per SHS and SCA policies
    • Establishes two personal goals annually
    • Ability to establish and maintain an effective working relationship with clients from various cultures and social economic backgrounds.
  • Professional relationships
    • Ability to establish and maintain an effective working relationships with other members of the staff, outside agencies and institutions, and the general public.
  • Other Responsibilities
    • Ability to plan and organize work, prepare records and reports, set priorities, and maintain a caseload in an effective and timely manner with minimal supervision.
    • Maintain confidentiality as required by regulations.
    • Acts as liaison to other human service agencies for coordination of services.
    • Performs other related duties as required.
  • Supervision
    • Supervised by: SCA Director and Case Management Supervisor
      • 1. Through weekly calendar summaries
      • 2. Through monthly Client Chart reviews
      • 3. Through Annual Employee Evaluation
      • 4. Through monthly Client Satisfaction Surveys

 

Qualifications

  • Bachelor's degree in an approved human service field or a Bachelor's Degree with 18 college credits in approved fields required.
  • One year direct experience in case management or counseling in drug and alcohol setting required.


Licensure, Certifications, and Clearances:

  • Driver's License
