The Behavioral Health Care Manager (BHCM) serves as an integral part of the youth wellness team establishing a therapeutic relationship with patients and families who meet the criteria for enrollment into the integrated behavioral health program. Relay information to primary care physician. BHCM provides comprehensive person-centered community-focused interventions for enrolled patients. In collaboration with the primary care provider and the psychiatric consultant, the BHCM coordinates the physical and mental health care plans for an assigned patient caseload.


  • This position will have a pediatric focus, working with a team of professionals that include the program coordinator, health coach, nutrition, and physical activity instructor
  • Demonstrates effective problem solving, and ability to communicate verbally and in writing, and a willingness to adapt to changing environments.
  • It is the duty of each employee to recognize that the interpersonal skills used in accomplishing the work assignments are as important as the work tasks themselves. Service to our customers is not an adjunct to our job functions, but an integral part of our functions. It is, therefore, the employee's responsibility to demonstrate the highest degree of professionalism by:
  • Facilitate treatment plan changes as needed for patient care, in collaboration with both the primary care provider and psychiatric consultant. Utilize weekly case reviews to gauge progress, and seek additional case review times as necessary.
  • Document telephone, in-person, and consultative care encounter within the EMR.
  • Evaluates and utilize behavioral health registry within Epic EMR. Utilize registry and clinical decision support to identify new cases, as well as articulate progress with known patients.
  • Support medication management with a focus on adherence monitoring, side effects, and effectiveness of treatment.
  • Track treatment response and monitor patients throughout the provision of care. Communicate response to care to the primary care provider and psychiatric consultant in weekly case reviews.
  • Ensures 'warm handoff delivery and care coordination at all levels of care.
  • Coordinates referral patterns and community resource use for the service of the patient population.
  • Provide patient education about care plan needs and available treatment options.
  • Screen and assess patients for common mental health and substance abuse disorders.
  • Evaluates and ensures program development and growth, including financial sustainability.
  • Coordinates physical and mental health care for patients on an assigned caseload.


  • Bachelor's Degree in a related field, including mental health counseling, marriage or family therapy, social work, nursing, or psychology.
  • Licensure in good standing in the field of service.
  • Experience with common mental health disorders.
  • Knowledge of evidence-based psychosocial treatments and brief behavioral interventions, including motivational interviewing and behavioral activation.
  • Knowledge of psychopharmacology as appropriate for the primary care settings.
  • In-depth knowledge of the electronic medical record systems, including the provision of protected services within an EMR.
  • Ability to use Microsoft Office products and other software applications.
  • Must have own vehicle for daily travel in the assigned service region.  Must maintain and provide proof of valid driver's license.  Must maintain own automobile coverage as required by law.
  • Evidence of additional training in behavioral health or integrative services preferred.
  • 10 years providing service to behavioral health populations preferred.
  • Knowledge of care coordination in a large health system preferred.
  • Pediatric focus, TIC, overweight/obese prior experience preferred.
