Responsible for maintaining the integrity of the medical records file room and the patient chart by following the approved policies and practices of the agency.


  • Retrieve charts on request for staff and responds to internal telephone requests for information.
  • Purge discharge charts according to procedure.
  • Inquire and update database for off-site storage.
  • Focus on customer service and continually strive to perform the duties of their job in a manner that will result in optimal patient satisfaction.
  • Prepare charts for external requests for PHI prior to review by Compliance Department
  • Retrieve copies of information as required for billing, audits, on-site surveys and other legal matters.
  • Process discharge charts according to procedure.
  • Attend compliance training and adheres to the organization standards of conduct, policies and procedures.
  • Maintain the integrity/security of the Medical Records department and the patient charts.
  • Responsible for electronic medical record retrieval/printing from the Home Care automated system to fulfill requests from 3rd party payers and other governing bodies.
  • Identify, develop and participate in process improvement opportunities within the home health agency that will enhance the quality of the services we provide.
  • Oversee Chart Facilitators daily activities in the absence of Medical Records Supervisor and ensure continued efficient department operations.
  • Alphabetize and file all patient documentation using approved chart order.
  • Perform duties and job responsibilities in a fashion which coincides with the service management philosophy of UPMC/Jefferson Regional Home Health, L.P. towards patients, visitors, staff, peers, physicians and other departments within the organization.
  • Follow Medical Records department policies and procedures.



  • High school diploma or GED is required.
  • Some knowledge of medical terminology preferred.
