Placement Specialist--Harrisburg

Facilitates and conducts searches for post-acute care facilities. Collaborates with the Care Management Manager, Care Manager and Social Worker to provide timely placement information to third party payors to ensure certification/approval of post discharge services. Communicates and coordinates placement availability to the Care Management Department, physician, and patient care services. Serves as a knowledge resource to interpret/negotiate with state, local, and federal agencies around placement request processes.


  • Performs other duties as assigned by the Director, Supervisor or Manager of the Department.
  • Xeroxes and faxes materials as required. Ensures maintenance of equipment
  • Maintains appropriate office supply quotas and routine ordering when needed.
  • Initiates and maintains accurate filing system.
  • Responsible for correspondence and follow-up related to resource utilization activities.
  • Functions as payor specialist as required.
  • Maintains, updates, and disseminates placement grid
  • Obtain Durable Medical Equipment, transportation, or other post-acute services as identified from the Care Management staff
  • Completes arrangement and transfer process within timeframe
  • Communicates all necessary information to Payor Specialist regarding placement and transportation as needed.
  • Continuously updates provides prompt feedback regarding placement determinations to Care
  • Communicates required information, identifies forms and requirements for all potential facilities, completes all forms appropriate for placement within time frame to prevent avoidable days, sends documents required to facility and documents appropriate information in Soft Me.
  • Initiates referral to post-acute service facility, documents in Soft Med
  • Identifies post-acute needs appropriate for patient/family and within their location and patient's provider network using the following criterion: Geographic location, Facility/service able to meet patient care needs & in-network with patient's insurance
  • Identifies patients requiring investigation of all potential placement needs from Care Management staff
  • Establishes and maintains relationships with outside resources and serves as a community resources to others
  • Understands the intricacies and can interpret/negotiate with individual payor, state, local, and federal agencies to optimize placement of patients in the most appropriate setting. Align needs of patients with placement options that are consistent with the desired quality and cost targets; such as, return to extended care facilities, assisted living, or reintegration into the home/community for patients.
  • Conducts searches for placement facilities and provides options for patients and families. Serves as a knowledge resource to interpret/negotiate with state, local, and federal agencies around placement request processes.



  • High School degree or GED required.
  • Will have completed internal or external medical technology course within six months or relevant clinical experience.
  • Associate or advanced degree preferred or LPN licensure or BSW
  • Two to three years experience in a health care setting
  • Knowledge of third party reimbursement or utilization management skills required plus strong knowledge of medical terminology
  • Experience with office machines (i.e. copier, fax, and telephone) computer terminal/personal computer, software (i.e. word processing, spreadsheets, etc.)

Licensure, Certifications, and Clearances:
Clearances must be dated within 90 days

  • Act 33 with renewal
  • Act 34 with renewal
  • Act 73 FBI Clearance with renewal
