UPMC Home Health is hiring a Chart Facilitator!

This is a Full Time position Workind 8A-430P. The office is located in Seven Fields, PA.

Responsibilities:

  •  Scan client documentation into the electronic medical record (One Content).
  •  Gather documentation daily for scanning from other departments.
  •  Organize and prep batches for scanning.
  •  Perform QA (Quality Analysis) on each scanned batch.
  •  Work the ADI Queue in One Content.
  • Process (check in) Face-to-Face documents in NetSmart and scan into One Content.
  • Shred saved scanned batches after five weeks.
  • Enter discharge chart information into Netsmart for storage at ACCESS.
  • Responsible for electronic medical record retrieval/printing from Home Care automated system to fulfill requests from 3rd party payers and other governing bodies.
  • Secure documents from daily SQL Mail.
  •  Work the Referral Integration email regarding Face to Face documents checked into Netsmart but not scanned into One Content.
  • Prepare charts for external requests for PHI prior to review by Compliance Department.
  •  Attend compliance training (as necessary) and adheres to the organizational standards of conduct, policies and procedures. 
  •  Focus on customer service and continually strive to perform the duties of their job in a manner that will result in optimal patient satisfaction.
  • Follow Medical Records departmental policies and procedures.
  • Identify, develop and participate in process improvement opportunities within the home health agency that will enhance the quality of the services we provide.
  • Maintain the integrity/security of the Medical Records department.
  • Oversee Chart Facilitators daily activities in the absence of the Medical Records Supervisor and ensure continued efficient departmental operations.
  • Perform duties and job responsibilities in a fashion which coincides with the service management philosophy of UPMC Home Healthcare towards patients, visitors, staff, peers, physicians and other departments within the organization.
  • Retrieve charts on request for staff and respond to internal telephone requests for information.
  • Retrieve copies of information as required for billing, audits, on-site surveys and other legal matters.
  • Inform the Medical Records Supervisor of needed supplies.

 

Qualifications

High school diploma or GED is required.

Some knowledge of medical terminology preferred.

Licensure, Certifications, and Clearances:

  • Act 34
