UPMC Family Hospice is hiring a Home Health Aide to support patients in their homes in Fox Chapel, Shaler, Gibsonia, and the Surrounding Area's!
This is a Full Time position working Monday-Friday, 8A-430P.

Responsibilities:
- Provide safe and direct personal care to patients in the home setting under direct supervision of appropriate professional.
- Attends compliance training and adheres to the organization standards of conduct, policies and procedures.
- Communicate appropriately with the interdisciplinary home care team in a timely manner, reporting any changes in patient's condition to appropriate professional staff.
- Participate in all required agency meetings.
- Identifies, develops and participates in process improvement opportunities within the home health agency that will enhance the quality of services we provide.
- Performs duties and job responsibilities in a fashion which coincides with the service management philosophy of the organization towards patients, visitors, staff, peers, physicians and other departments within the organization.
- Maintains and completes documentation accurately and promptly while in the patient?s home and within the agency and regulatory standards.
Qualifications
High School Diploma or GED - Required
Must have successfully completed a training and competency evaluation program as specified in 484.80 (Home Health) and/or 418.76 (Hospice) as applicable: Condition of participation: Home Health aide services.One (1) year experience - Preferred; Active employment within the past 24 months as a Certified Nurse Aide in good standing.
Licensure, Certifications, and Clearances:
CPR required based on AHA standards that include both a didactic and skills demonstration component within 30 days of hire
