Description

Assist in accomplishing program goals for consultative, advocacy and supportive services for technology assisted children and their families throughout the Commonwealth of Pennsylvania.

* Prior to the COVID-19 Pandemic, the nurse social worker & therapists conducted in-home visits.  In-home vists remain suspended and video visits are used as substitutes.  There is a likelihood that home visits will resume at some point.*
 

Responsibilities:

  • Networks with other professionals working in related or complimentary fields to share ideas and resources.
  • Evaluates home care funding for each patient and family.
  • Exercise professional nursing judgement in the assessment of the child and the family.
  • Maintains records and statistics are required by the Department of Health and the TACHP.
  • Provides education regarding psychosocial issues for technology assisted children and families to other healthcare professionals.
  • Demonstrate sound professional decision making skills during formation of plan of care.
  • Establishes priorities of case management based on diagnosis and family needs.

 

Qualifications

  • Required level of education:Bachelor's degree.
  • Area of education: Nursing.
  • Required level of experience: 3 years.
  • Area of experience: Pediatrics, Home Health or related experience.  Case Management experience preferred.


Licensure, Certifications, and Clearances:

  • Registered Nurse (RN)
  • Act 31 Child Abuse Reporting
  • Act 33
  • Act 34
  • Act 73 FBI Clearance
