Description

Family Hospice is hiring a full-time home health aide to work daylight shifts in the north hills location! Apply today to learn more about this role! 


Provide designated care under the direction of the primary service provider in the home in order to achieve identified goals. Function as an extension of skilled nursing services and/or rehabilitation services in the home.

Responsibilities:

 

  • Focuses on customer service and continually strives to perform the duties of their job in a manner that will result in optimal patient satisfaction.
  • Proven ability to facilitate change.
  • Provide safe and direct personal care to patients in the home setting under direct supervision of appropriate professional.
  • Attends compliance training and adheres to the organization standards of conduct, policies and procedures.
  • Communicate appropriately with the interdisciplinary home care team in a timely manner, reporting any changes in patient's condition to appropriate professional staff.
  • Participate in all required agency meetings.
  • Identifies, develops and participates in process improvement opportunities within the home health agency that will enhance the quality of services we provide.
  • Performs duties and job responsibilities in a fashion which coincides with the service management philosophy of the organization towards patients, visitors, staff, peers, physicians and other departments within the organization.
  • Maintains and completes documentation accurately and promptly while in the patients home and within the agency and regulatory standards.

 

Qualifications

  • High School Diploma or GED - Required
  • Must have successfully completed a training and competency evaluation program as specified in 484.80 (Home Health) and/or 418.76 (Hospice) as applicable: Condition of participation: Home Health aide services.
  • One (1) year experience - Preferred; Active employment within the past 24 months as a Certified Nurse Aide in good standing.


Licensure, Certifications, and Clearances:
CPR required based on AHA standards that include both a didactic and skills demonstration component within 30 days of hire
