Admissions Coordinator (Canterbury Place)

UPMC is hiring an Admissions Coordinator. 

This is a Full-Time position. The Office is located at Canterbury Place in Lawrenceville, PA.

Responsibilities:

  • Manages the facilities' referral waiting list on a daily basis, to ensure shortest wait time possible, communicating any changes to the CAO admission assistants and liaisons and recording and updates on the patient communication record and the daily referral tracking tool.
  • Received and reviews resident referrals from hospital discharge planners, obtains clinical and financial information, verifies insurance/payer, appropriately triages, and provides an accept/deny decision in a timely manner according to the CAO process.
  • Under the direction of the manager, provides referral tracking data for the monthly consolidated tracking tool used by UPMC Senior Communities Administration.
  • Maintains referral tracking log and patient communication record (front of file) and reports daily updates to the CAO assistants and liaisons.
  • Communicates the need for follow up on residents transferred to the hospital to the nurse liaison.
  • Consistently works cooperatively and pleasantly with all staff, residents, family members, and referral sources.
  • Collaborates with the nursing facilities' admission assistant to maintain and ensure accuracy of real time bed availability and communicates this information to the clinical nurse liaisons and referral discharge planners as needed.
  • Each coordinator is responsible for bed management in two of the four UPMC Skilled Nursing Facilities as assigned.
  • Coordinates and chairs daily morning census calls according to assigned buildings.
  • Coordinates admission of residents with assigned facilities according to the CAO process.
  • Manages the ADC and appropriate CMI during the referral acceptance process.
  • Completes preadmission data entry of demographic and preliminary financial information
  • Effectively communicates and prioritizes onsite requests for clinically and psychosocially complex referrals to nurse liaison according to the CAO process.

 

Qualifications

  • Associate's degree or completion of secretarial program is preferred.
  • High school diploma or equivalent.
  • 3 years of administrative support experience required.
  • Proficient with personal computers, word processing, spreadsheet and data base management, email, and electronic scheduling (Microsoft Word, Excel, Access, Powerpoint) and use of Internet is required.
  • Excellent organizational skills and office management experience preferred.Courtesy, diplomacy, service excellence, confidentiality and good judgment are inherent in the job and are pre-requisites.
  • Ability to communicate with all customers including residents, family members, visitors and staff is required.
  • Ability to exercise judgment in evaluating problem situations and take initiative in referring such situations to the appropriate person is required.
  • Ability to demonstrate composure at all times exhibiting courtesy, respect and a helpful positive attitude at all times in face-to-face and telephone interactions with customers is imperative.
  • Ability to prepare accurate typed material and complete assignments with accuracy and attention to details in a timely manner is required.
  • Physical activities essential to satisfactorily perform this position involve; balance, crouching, grasping, fingering, kneeling, lifting 35-50 pounds without assistance, pulling, pushing, reaching, standing, and stooping.Sensory and communicative activities essential to satisfactory performance in this position include; hearing seeing, smelling, speaking, tolerating an above average level of stress and maximum interaction with people.
  • Strong time management skills and the ability to prioritize multiple tasks are required.
  • Possibly be exposed to inside environmental conditions, physical hazards and chemical hazards.
