Description

This new full-time position performs a vital role by supporting GLO Harrisburg’s HIV prevention efforts and medical case management service for young LGBTQ+ men of color and transgender women of color.

Please note, this staff member must have the ability to work afternoon and evening hours at GLO Harrisburg (until 8:00 PM). This staff member will also work at the REACCH Program (located 1 mile away from GLO) 1 or 2 days/evenings per week.

Purpose:
This role directly supports the GLO Harrisburg team by delivering high-quality HIV Navigation. Services to individuals at a high risk of acquiring HIV. This role will include performing rapid HIV testing, collection of STI tests, and linking individuals to the REACCH Program’s pre-exposure prophylaxis (PrEP) services. This role may accompany clients to their medical appointments at the REACCH Program, and/or other community agencies.

Responsibilities:

  • Provides HIV Navigation Services to approximately 40-50 clients in the form of pre-exposure prophylaxis (PrEP) and post-exposure prophylaxis (PEP) referral and linkage, and other risk-reduction activities.
  • Follow-up with clients to ensure they have access to PrEP or PEP medications and encourage adherence.
  • Provide rapid HIV and STI testing on-site at GLO Harrisburg.
  • Serve as a resource to GLO staff and participants for community resources and referrals.
  • Participate in community outreach events for education and HIV testing alongside the GLO Harrisburg and the REACCH teams.
  • Maintain a caseload of approximately 10 medical case management clients.
  • Perform and follow-up according to Part B Ryan White medical case management standards for these clients.
  • Ensures all clients applying for housing assistance meet eligibility and financial requirements

 

Qualifications

  • MSW degree from a CSWE accredited School of Social Work required.
  • Experience serving marginalized populations, such as LGBTQ+ and/or BIPOC clients preferred.
  • Experience in the HIV/AIDS field preferred.
  • Experience providing case management and knowledge about community resources in Dauphin county preferred.
  • Bilingual preferred.


Licensure, Certifications, and Clearances:
Current / active Social Work License required or successful pass of the social work licensing exam within 9 months of employment.

Clearances must be dated within 90 days
 

  • Clinical Social Worker (CSW) OR Licensed Social Worker (LSW)
  • Act 33 with renewal
  • Act 34 with renewal
  • Act 73 FBI Clearance with renewal
