Coordinate and provide safe patient care on any shift.

Responsibilities:

  • Perform reviews with various managed care companies, documenting interactions and clinical information given to ensure no gap is the covered days. Perform daily review of patients by payor class, communicates this data thru established mechanisms to the billing office. Present notification to the billing office of changes in payor source, admission, transfers and discharges.
  • Complete tracking forms; reentry and discharge. Assist the SNM in the orientation, training, development and evaluation of nursing employees on MDS, care plans and documentation requirements. Manage data collections for Case Mix groupings quarterly for MA residents and annually for residents.
  • Coordinate and formulate comprehensive plan of care with all disciplines. Complete quarterly review on every resident Maintain restraint log on every resident on admission and quarterly and with changes in Level of Care.
  • Manage timely electronic submission of data to Harrisburg weekly, monthly, quarterly and annually. Monitor specific data that affects CMI and ultimately reimbursement Assess Level of Care on every resident on admission and then on a continuing basis
  • Consistently work cooperatively with residents, staff, physicians, families, consultant personnel and ancillary service providers. Assume accountability for data contained in the employee's handbook.
  • Follow established performance standards and performs duties according to nursing service policies and procedures when acting in a charge/staff nurse role. Follow Resident Rights policies at all times.
  • Accept assigned duties in a cooperative manner.
  • Complete MDS 2.0 and Assessment on admission, annually and with every change in Level of Care. Complete section S of MDS2.0, RAPS, Triggers on admission, annually and with very change in Level of Care. Initiate and complete Plan of Care on admission, quarterly and with changes in Level of Care.
  • Tally and notify billing office of bed hold status for affected residents, and timely notification upon return Knowledgeably and competently performs basic nursing skills. Personally obtain continuing education related to the field of long-term care nursing.
  • Notify families/resident when Plan of Care conference is scheduled. Coordinate and facilitate the Interdisciplinary Plan of Care conference on every resident. Monitor data entered into computer under MDS and Plan of Care for accuracy and completion.

 

Qualifications

Experience as an RNAC preferred

1 year experience in long term care as a charge nurse required

Physical activities essential to satisfactorily perform in this position involve: balancing, crouching, fingering, grasping, kneeling, lifting 35-50 pounds without assistance, pulling, pushing, reaching, standing and stooping

Sensory and communicative activities essential to satisfactory performance in this position include: feeling, hearing, seeing, smelling, speaking, tolerating an above average level of stress, and maximum interaction with people

Strong time management skills and the ability to prioritize multiple tasks are essential

Flexibility with schedule and willingness to work on the unit as needed will be strongly desired

Possibly be exposed to inside environmental conditions, blood borne pathogens, physical hazards and chemical hazards

Licensure, Certifications, and Clearances:
CPR required based on AHA standards that include both a didactic and skills demonstration component within 30 days of hire
