This role will support both the hematology care team at UPMC Children’s Hospital of Pittsburgh (CHP) and the care team at The Hemophilia Center of Western Pennsylvania (HCWP).  The individual will have access to both EMRs, email systems, and will spend roughly equivalent time at each institution.

This position is Flex Full Time and may require availability for Full Time hours as needed.

Responsible and accountable for coordination of patient care through all phases of process. Works closely with physicians, patients and families to provide technical knowledge of the process and guidance on appropriate actions relative to insurance coverage, necessary ancillary testing and follow up care. Acts as a technical resource to multidiscplinary team.


  • Clinical areas of coverage (with mentorship from hematology team):
    • Bleeding disorders
    • Clotting disorders
    • Other
  • Daily Responsibilities
    • Assist with scheduling, especially for new patients / Communicate with schedulers / front desk
    • Respond to phone calls / document phone messages in Cerner and Centricity
    • Provide support and clinical recommendations to patients and families
    • Provide procedure plans
    • Complete Medication orders / refills
    • Enter / send lab orders
    • Follow up on lab results / make any needed phone calls
    • Communicate hemophilia plans of care to providers and families
    • Complete required paperwork for families
    • Fax paperwork / lab results as required
    • Coordinate outpatient follow-up after hospital discharge among different specialties
    • Coordinate infusions, home care and pre-authorizations for patients
    • Check Cerner communication from different specialties regarding bleeding/clotting patients for coordination of care
  • Coordination with Hematology Providers and HCWP staff: 
    1. Provide single point of access for patient into health care service.
    2. Facilitate communication between CHP and HCWP
    3. Meet patient families, provide contact information, build relationship
    4. Work with attendings, nurses and APP providers that follow hematology patients
    5. Work with clinic “doc of the day”, clinic captain, HCWP staff and Hematology attending/Fellow to coordinate urgent clinic referrals
    6. Coordinate with inpatient Hematology attending for education of new patients and to coordinate follow up after discharge
    7. Coordinate urgent visits between HCWP patients and ED’s (CHP and referral catchment area)
    8. Help to ensure that new patients are seen within the appropriate time frame (urgent versus nonurgent) and with the appropriate clinic.
    9. Coordinate appointments with other divisions / departments
    10. Coordinate long-term outpatient follow up (labs, prescriptions, future visits)
    11. Coordinate patient care and referrals to research team at HCWP
    12. Review educational material with patients
    13. Communicate medication / lab/ clinic plans with family
    14. Assist with home care needs
    15. Coordinate and facilitate adequate follow-up and medication adherence/update
    16. Formulate quality improvement protocols/projects
  • Hematology clinics: 

    1. Attend a subset of hematology clinics:
      1. See patient first
      2. Update eRecord with medications, allergies, etc.
      3. Educate families as needed
      4. Draw from mediports and give immunizations as needed
      5. Coordinate follow up care after provider has seen patient
    2. Ensure that patients are appropriately scheduled for each subspecialty clinic
    3. Collect outside clinical and laboratory information prior to clinic visit.  Review with provider as needed prior to clinic visit.
  • Coordination with families: 
    1. Meet new families in hospital and in clinic
    2. Primary phone contact for families
    3. Coordinate outside laboratory studies and follow up routine results with families
    4. Coordinate appointments
  • Coordination with outside providers: 
    1. Work with PCP to obtain initial referral information (clinical and labs)
    2. Work with PCP for follow up lab studies
    3. Serve as a resource for PCPs for new referrals
  • Educational and quality initiatives: Develop educational materials for common hematology diseases. Educate schools for special needs of hematology patients.



  • Nursing Degree Required, BSN strongly preferred.
  • 3 years Clinical nursing experience.

Licensure, Certifications, and Clearances:

  • Registered Nurse (RN)
  • Act 33 with renewal
  • Act 34 with renewal
  • Act 73 FBI Clearance with renewal
