Description

UPMC Home Health is hiring a Full-Time Clinical Coordinator to join our team in West Mifflin office!

This is a Monday-Friday Daylight shift.

Position requires rotating weekends and holidays + on call requirements. 


Responsibilities:

 

  • Builds and mends relationships among colleagues within the framework of a healthy workplace. Role models keen cultural awareness including preventing and addressing horizontal violence and impairment. Maintains a conscious balance between work and personal life through prioritization and delegation of responsibilities. Extends trust by acknowledging the contributions of others; listens first, creates transparency in communications, confronts reality, and clarifies expectations.
  • Demonstrates good judgment and analytical ability to oversee and coordinate patient care in collaboration with the physician and the multidisciplinary health care team. Uses critical thinking skills to effectively advise and direct health care providers in their delivery of patient care while maximizing the efficient use of human and financial resources to achieve productivity standards.Responsive to all departments daily; accountable to establish professional and respectful interpersonal relationships.
  • Analytical ability necessary to address and improve clinical issues and assist with complex problem solving. Leadership ability to maintain standards of performance within a national context for professional nursing practice and comply with administrative and regulatory requirements. Superior interpersonal and communication skills sufficient to effectively create and maintain a positive, caring relationships with executives, managers, physicians, non-physician providers, ancillary and support staff, other departments, and patients/families in both written and verbal communication.
  • Transformational Leadership Independently suggests and supports changes within the department/organization Involves staff and key stakeholders to achieve optimal patient experience and efficient patient throughput. Uses appropriate interpersonal styles and techniques to gain acceptance of ideas or plans; modifying one's own behavior to accommodate tasks, situations and individuals involved. Supports staff during times of difficult transitions. Participates in supporting mechanisms and systems that ensure the provision of effective patient care while utilizing resources appropriately. Adhere to reporting elements and requirements set forth by all regulatory agencies for patient safety.
  • Identifies opportunities for clinical quality as well as workplace improvement. Applies evidence based practice as a regular aspect of professional practice
  • Facilitates teamwork among all departments involved in the patient care process. Seeks opportunities to share expertise with other members of the healthcare team within and beyond the clinical unit/department. Regularly seeks feedback on performance and applies continuous professional improvement. Supports continuous learning in the clinical area by sharing research and materials from professional organizations, presenting on the unit, supporting journal clubs, etc

 

Qualifications

  • Minimum of 2 years healthcare experience- required
  • Minimum of 1-year home health (or hospice) experience- preferredMinimum of 1 years progressive leadership experience with relevant clinical experience preferred
  • BSN preferred


Licensure, Certifications, and Clearances:
Current licensure as a professional nurse in PA_-RequiredCurrent Certification in basic CPR- RequiredCurrent driver's license in state of residence- RequiredCurrent auto insurance in state of residence- RequiredAct 73 required for Pediatric/OB caregivers- Required
