Restorative Coordinator LPN - Full Time - Cole Place - Senior Communities

UPMC Cole is looking for a full time Restorative Coordinator LPN to join the team on day shift.  This position will work one weekend per month.

The Restorative Nursing Coordinator may have multi- facility responsibility for the coordination and management of the Restorative Nursing Program. The RNC works collaboratively with the nursing department, the Rehabilitation Department and the RNAC office to coordinate resident needs with appropriate restorative nursing programs and to ensure all requirements are met according to state and federal regulations. The RNC collaborates with the RNACs to ensure all care plans are maintained and meet requirements. The RC demonstrates the clinical knowledge and skills necessary to perform assessments and develop plans of care to provide restorative nursing care that is individualized to the resident. The RNC directs restorative nursing assistants in providing and documenting the established plan of care.



  • Identify and initiate referral to Rehab for residents demonstrating need for PT, OT or SLT.
  • Coordinate the Restorative Care Planning process, initiating restorative care plans for all appropriate residents.
  • Under supervision of Registered Nurse, evaluates residents' restorative needs on admission, quarterly, and when referred by therapists, nursing or others.
  • Collaborate with Rehab to implement initial and annual training and competency evaluation of Restorative CNAs.
  • Collaborate with RNACs, nursing, rehab and other members of the interdisciplinary team to ensure the residents' needs are identified and appropriate measures are taken to meet them.
  • Monitor the delivery of restorative nursing care to all residents on restorative programs.
  • Maintain adequate inventory of department supplies as needed.
  • Promote a safe environment for residents and staff performing restorative nursing procedures including adherence to standard precautions, safety policies and procedures and safe practices for lifting and moving.
  • Coordinate and manage the Restorative Nursing Program for 1 or more Skilled Nursing facilities.
  • Meet regularly with Rehab to review residents and facilitate transition of residents through the different levels of Rehab and Restorative Nursing.
  • Initiate, monitor and maintain documentation for all phases of the restorative process for all residents, including assessment tools, care plans, communication forms, and documentation of daily program delivery (Restorative CNA documentation/RCNA).
  • Participate in facility care meetings as indicated, e.g., Resident Safety, Falls Risk reduction, Restraint Reduction, Rehab, CMI, QA and others as indicated.
  • Manage, schedule and supervise RCNAs, including managing performance, facilitating education and monitoring competency.
  • Design and implement individualized restorative plan of care for appropriate residents with review by Registered Nurse.
  • Monitor residents' response to restorative treatment, communicating the residents status to all concerned.
  • Monitor the care planning process for timely updates and revisions as indicated.
  • Identify residents demonstrating plateau of performance and facilitate continuation of maintenance level restorative care interventions.
  • Provide ongoing evaluation and revision of residents program as indicated.



  • 6 months of experience as an LPN.
  • LTC experience preferred.
  • Solid/Strong/Good or higher rating on most recent performance evaluation.
  • Completion of Aging Institute Geriatric Sensitivity Program.
  • Oversees Restorative CNAs

Licensure, Certifications, and Clearances:
CPR required based on AHA standards that include both a didactic and skills demonstration component within 30 days of hire. Must complete licensure examination within FOUR MONTHS of hire or within one year of graduation, whichever comes first. Though temporary permits are valid for one year from the date of graduation, not one year from issue of permit (or until the results of the examination are known at which time it becomes null and void), UPMC requires that GNs take the licensure examination within 4 months of their start date or they will be terminated or demoted from the GN position, at the discretion of the BU.

  • Basic Life Support (BLS) OR Cardiopulmonary Resuscitation (CPR)
