CTT Clinician III

Description

Under the direction of the CTT program coordinator, the CTT Clinician III provides comprehensive mental health treatment to residents of Allegheny County in community settings, including but not limited to, homes, hospital emergency rooms, personal care homes, etc. The CTT program operates 24 hours a day, 7 days a week, 365 days a year.

Primary responsibility is providing intense outpatient mental health treatment in a variety of settings to consumers with serious and persistent mental illness for whom traditional outpatient Mental Health services have not been effective. Responsibilities include clinical assessment, treatment planning, coordinating care in the community, interfacing with other community providers in the coordination of this care. This position serves as Clinical Lead for the CTT team, providing mentoring and clinical supervision to select team members and takes leadership role in ensuring fidelity to ACT/CTT model.

CTT at WPH is the highest level of outpatient care with opportunities to serve the neediest in the community through an evidence-based approach with potential for creativity and innovation. The ideal candidate will have knowledge in evidence-based practices in therapy, employment, and psych rehab. 

Responsibilities:

 

  • Manage crisis situations effectively using positive approaches.
  • Act as an advocate for the consumer and/or family and provides advocacy for consumers, including 302 an other involuntary commitment proceedings, guardianship, and custody proceedings.
  • Participate in working weekend rotation.
  • Support consumer's WRAP plan.
  • Collaborate with team members to provide needed services to coordinate with other community agencies.
  • Maintain accuracy and completeness of patient records in compliance with institutional standards.
  • Work efficiently and independently.
  • Provide supportive counseling to consumers to promote stability.
  • Provide insight & knowledge of insurance programs, processes and able to obtain authorizations as needed.
  • Participate in outreach visits to consumers in various housing/living situations.
  • Provide consultation and direct assistance to the DEC or community ER staff in the case of at risk consumers presenting for treatment, emphasizing safe diversion from inpatient stays in accordance with ACT models of care.
  • Provide quality care to consumers following the Assertive Community Treatment(ACT) Model of mental health care delivery.
  • Serve as primary case coordinator for a caseload of 10-15 individuals/families, yet may work with any member/family on the active caseload as needed.
  • Assist consumers to obtain and maintain culturally appropriate basic living needs through skills such as: food, medical care, recreation, education, and employment, through referral and assertive follow-up, advocacy, and problem resolution.
  • Support consumer and negotiates appropriate referral services.
  • May facilitate or co-lead small groups of consumers/or significant others to increase understanding of consumer issues and to improve coping skills and performance of activities of daily living.
  • Participate in and/or completes a comprehensive assessment of identified clients. This includes social history, psychiatric history, and mental status exams.
  • Complete consumer/family crisis assessments.
  • Participate in team meetings in a collaborative manner.
  • Provide services in a community and/or office setting.
  • Utilize specific techniques of motivational interviewing.
  • Provide follow-up services to consumers by phone.
  • Maintain a minimum productivity rate of 55%.
  • Responsible for learning, continuing education, and implementation of ACT standards of care.
  • Participate in 24 hour on call rotation.
  • Assist consumer in developing a crisis, relapse, and service plan.
  • Submit documentation that is professional in content and appearance by established deadlines.
  • Assist consumer to set goals and plan to meet those goals based on consumer's strengths, needs, interests and cultural preferences
  • Provide therapeutic interventions with fidelity to the ACT model of care and using evidenced based practices.
  • Under the guidance of the Director, and as delegated, the Clinician III will provide formal group/individual supervision at least once every 2 weeks and act as a representative accountable for the coordination of care and quality improvement (QI) functions of the team (including but not limited to Continued Stay Reviews, interagency meetings, ACT initial assessments and treatment plans, departmental regulatory compliance)

 

Qualifications

  • Masters Degree in Psychology, Counseling or Social Work AND four years of mental health experience, supervisory experience preferred.
  • In lieu of this, Bachelors Degree in Psychology, Counseling or Social Work WITH six years clinical experience WITH three years crisis experience will be considered.
  • Other fields of study may be considered with coursework of at least 12 credits in human services.
  • License in Social Work or in Professional Counseling preferred.
  • Work history that an in depth knowledge and proven skill at doing psychiatric assessments.
  • Experience in mental health crisis intervention, case management, or in home therapy preferred
  • Minimum of two years of administrative/supervisory experience preferred
  • Certified peer specialist supervisor (or obtain certification within 6 months of hire) preferred
  • Excellent, recovery-oriented interpersonal relations.
  • Ability to work shifts, weekends, holidays, and to assume on call responsibilities.
  • May in the course of their duties be in rooms where medications are stored or may transport medications when they transport patients. Transportation of medication is limited from one department to another.

Licensure, Certifications, and Clearances:
CCM training is required within 30 days of hire and annually thereafter

  • Basic Life Support (BLS) OR Cardiopulmonary Resuscitation (CPR)
  • Comprehensive Crisis Management (CCMC)
  • Driver's License
  • Act 34
