Supported Housing Coordinator

Supported Housing has 3 staff members who work with the treatment team to find housing that meets individuals' needs, and helps individuals gain the resources needed to maintain success in their current/new housing situation. Staff report to a supervisor but work independently for most of their shift collaborating with each other for additional resources or ideas for individual needs. The current team project is to create a living document with a go-to list of resources not only for Supported Housing but also Service Coordination for any situation.

The ideal candidate for this position will have knowledge of housing resources. The hire for this position should have the ability to problem-solve and be able to research other housing, transportation, financial, clothing, health care, and food resources in the community as needed. 

The dress code is casual and parking is available near the building. This is a Monday through Friday position, 830a-4p. 



  • Documentation ActivitiesAssesses and documents consumer?s functioning level and updates this at least annually.Develops goal and service plans based on the Skill Assessment and input from the consumer.Writes thorough, informative and complete progress notes.Develops new forms and processes as necessaryWorks on QA initiatives as it pertains to service delivery
  • Respects consumer?s personal beliefs and practices. Remains calm, cooperative and supportive, especially in emergency and unsettling circumstances.
  • Counseling ActivitiesEstablishes and maintains effective working relationships with all MYCS consumers, striving to exceed their expectations for service, quality and delivery.Meets with assigned consumers to discuss progress; address set backs and assists consumers with identifying their treatment needs.Assists consumers with identifying and implementing long and short-term goals; communicates goal progress to consumer on and provides feedback when needed.Provides education and guidance to assist consumers with making life choices. Uses crisis intervention techniques to effectively de-escalate consumer behavior and avoid potentially dangerous situations.
  • Works independently, under stress and change while performing multiple tasks simultaneously.
  • Casework ActivitiesAssists consumers with scheduling and attending psychiatric, medical and dental appointments, assuring that appropriate documentation is maintained and filed, determines the need for and service delivery of emergency care, utilizing appropriate and available methods of payment; and education regarding healthcare and illness, including the benefits of education and medication.Assists consumers with applying for appropriate financial assistance, developing a budget, establishing checking/savings accounts, and preparing tax forms, rebates, etc.Educates and assists consumers with daily living skills such as menu planning, meal preparation, household chores, laundry and personal hygiene.Assesses, educates, and assists consumers with public transportation, which includes accompanying them on the bus when necessary.
  • Cooperates and remains flexible in work schedule adjustments to meet the needs of program operations
  • Operates independently in the community, interfacing with medical, psychiatric, and other MH professionals as well as landlords and housing representatives.
  • Casework Activities continued:Provides orientation to the local community with location of pharmacies, grocery stores, recreational facilities, hospitals, doctors, and dentists.Educates consumers on emergency procedures such as fire safety, natural disasters, loss of utilities, violence and abuseTeaches and encourages the development of social skills through role-plays and participation in recreational activities.Assists with securing housing as well as teaching skills to complete housing applications and interface with landlordsCommunicates with treatment team members and keeps members informed of consumer?s progress.Attends treatment team and all other scheduled meetings as deemed appropriate. Attends all monthly staff meetings.? Provides internal and external liaison to appropriate service providers in accordance with the consumer?s service plan.
  • Provides quality services at all times to consumers regardless of medical, psychiatric and/or behavioral issues.



  • Bachelors degree in Social/Human Science field
  • Driving is an essential job responsibility, therefore must possess a valid PA driver's license and be accepted by the agency?s vehicle insurance as a valid driver.

Licensure, Certifications, and Clearances:

  • Basic Life Support (BLS) OR Cardiopulmonary Resuscitation (CPR)
  • Comprehensive Crisis Management (CCMC)
  • Driver's License
  • Act 31 Child Abuse Reporting with renewal
  • Act 33 with renewal
  • Act 34 with renewal
  • Act 73 FBI Clearance with renewal
