Description
UPMC University of Pittsburgh Physicians Division is currently hiring a Regular, Full-Time Diabetes Educator to support the Department of General Internal Medicine located in West Mifflin, PA! This position will work a Monday - Friday daylight schedule, no weekends or holidays required! Travel to the Shadyside and Oakland offices will be required.
Duties for this position include but are not limited to, telephone triage, adjusting insulin, recommending diabetes medications, case management and refilling prescriptions.
Purpose:
Provides diabetes education to patients referred to the Diabetes Education Network in outpatient, inpatient or community setting. Provides support to the other functions in Endocrinology i.e., specialized procedures.
Responsibilities:
- Provides education according to the National Standards for Diabetes Self-Management Education in association with all relevant aspects of health care for the person with diabetes.
- Participates in achieving/maintaining American Diabetes Association Recognition for Health System's educational programs in an outpatient, inpatient or community setting.
- Instructs patients and family members/significant others in the implementation of Healthy lifestyles, Blood glucose testing, Medication management, Risk reduction, Insulin administration, Insulin pumps.
- Assesses diabetes patients' educational and management needs and creates a care plan in an outpatient, inpatient or community setting.
- Manages on-going education/care for patients (consulting with physician as appropriate) by answering questions, reviewing home glucose monitoring records, reviewing diabetes medications accordingly and monitoring glycemic control.
- Educators are expected to provide training and updates on diabetes education to staff.
- Accurately documents all patient interactions and ensures appropriate and complete billing for diabetes education services.
- Adjusts medication/insulin doses according to established approved protocols when appropriate.
Qualifications
- Bachelors Degree in Nursing or related health care field preferred.
- 2 years clinical experience is required.
- Previous diabetes education experience preferred
Licensure, Certifications, and Clearances:
Licensure as a registered nurse or dietitian, clinical psychologist, occupational therapist, physician assistant, exercise physiologist, pharmacist, or physical therapist
- Act 34
