Care Coordination Assistant

Provides support to the Care Coordination Department assisting with the discharge and utilization management process. Performs tasks as delegated by the Care Coordination staff during the initial and ongoing phases of discharge planning for management of patients across the continum. Provide ongoing follow-up, basic interviews with patients including enrolling and or referring patients to community resources. Act as a patient advocate and liaison between the patient/family and community service/resource agencies to improve health of patients and families. Assist patients with completing applications, registration forms, and connect with transportation resources. Schedule follow-up primary care provider appointments, provide appointment reminders, and make follow-up phone calls when appropriate. These duties will include both clerical and non-clerical functions that are within the scope of non-licensed personnel within the inpatient and ED/Observation Unit setting. Ordering durable medical equipment, arranging transportation, setting up post discharge appointments, issuing the Important Message from Medicare letters to patients, providing Advanced Directive Information and assisting patients with completion as needed. These duties will include both clerical and non-clerical functions.

  • High School Diploma/GED required.
  • Customer service and/or previous work in clinical setting preferred.
  • Understand the community served and be knowledgeable about community resources appropriate for needs of patient/families.
  • Ability to provide information to patients as delegated while monitoring and reporting on any need for follow up by licensed staff.
  • Basic computer skills including Word, Excel, and Outlook.
  • Excellent communication, organizational and prioritization skills.
  • Motivational interviewing to support challenging discharge planning situations.
  • Ability and willingness to provide emotional support, encouragement and motivation to patients as well as the ability to initiate and maintain positive working relationships with staff and other organizations.
