UPMC complies with all governmental requirements related to local, state, and federal COVID-19 vaccination for employment. Community Engagement Coordinator- Northeast, Pennsylvania

Description

Community HealthChoices (CHC) is Pennsylvania's managed care long term services and supports (LTSS) program serving seniors and individuals with physical disabilities in the Commonwealth who are covered by Medicare and Medicaid.  Looking for candidates throughout the Northeast region of Pennsylvania.


The Community Engagement Coordinator assist with oversight of the CHC Community Engagement Programs. Provides training, assist team members in resolving problems, facilitate decision making, and escalate issues when necessary to ensure timely and accurate responses between DHS and the Health Plan consistent with the Pennsylvania Department of Human Services (DHS) and/or Centers for Medicare and Medicaid Services (CMS) regulations. The Community Engagement Coordinator plays a vital role in developing and mentoring staff, including support with oversight of staff performance. and community stakeholder outreach in assigned zones.

Responsibilities:

 

  • Contractual meetings adherence, committee recruiting and monitoring, submission of all required DHS materials for review in a timely manner. Collaboration with internal partners on health-related subject matter experts on design, development and delivery of health awareness information. Identifying UPMC resources, community and provider partners who can support UPMC Health Plan's goals for improving service and the quality of care provided to the membership.
  • Development of project plans and monitors status of assigned projects in assigned region. Provides written summaries to Manager of status on a frequent basis. Provides support to initiatives as requested. Collaboration with internal departments in the development of plans involving community and provider partners to improve service and the quality of care provided to the membership. Serves as a resource to internal departments. Plans, coordinates and implements community health promotion events. Adheres to Department of Public Welfare, Pennsylvania Insurance Department or other governmental or regulatory requirements for participation in community events or incentives.
  • Identifying community leaders, organizations and providers who provide services to consumers who may be eligible for UPMC Health Plan products. Provides information on UPMC products and links to appropriate Health Plan departments. Completion in a timely manner of all required Department of Public Welfare, Pennsylvania Insurance Department or other governmental or regulatory reports on community involvement or events.
  • The development of training materials related to community relations. Participates in education programs related to scope of community relations department for internal staff and external groups. Evaluates ROI on all key outreach initiatives and programs in assigned zones.

 

Qualifications

Bachelor's degree in a health-related discipline or in social services, communications, business, and 5 years' experience working in health care, community-based organization, or customer service. Master's degree in public health preferred. Prior experience leading/ supervising staff preferred. Superior computer skills are a given, with proficient knowledge in Microsoft Office products. Ability to work in a fast-paced environment a must. Ability to manage multiple tasks and projects, and forge strong interpersonal relationships within the department, with other departments, and with external audiences. Ability to plan, implement and evaluate community health promotion events required. Working knowledge of health behavior change and motivational strategies preferred. Travel regionally and Statewide required.
