UPMC Home Health is hiring a Home Care Aide to join its team and support Mercer and Lawrence Counties!

This is a Part Time position. Typically the schedule will be 2 days one week, 3 days the next. On Call/Weekend/Holiday availability required.

Responsibilities:

  • Communicates appropriately with the interdisciplinary home care team in a timely manner, reporting any changes in patient?s condition to appropriate professional staff.
  • Attends compliance training and adheres to the organization standards of conduct, policies and procedures.
  • Focuses on customer service and continually strives to perform the duties of their job in a manner that will result in optimal patient satisfaction.
  • Provides safe and direct personal care to patients in the home setting under direct supervision of appropriate professional.
  • Performs duties and job responsibilities in a fashion which coincides with the service management philosophy of the organization towards patients, visitors, staff, peers, physicians and other departments within the organization.
  • Identifies, develops and participates in process improvement opportunities within the home health agency that will enhance the quality of services we provide.
  • Proven ability to facilitate change.
  • Maintains and completes documentation accurately and promptly while in the patient's home and within the agency and regulatory standards.
  • Participates in all required agency meetings.

 

Qualifications

  • High School Diploma or GED - Required
    Required: successfully completed a training and competency evaluation program as specified in Home Health and/or Hospice as applicable:  Condition of participation: Home Health aide services.
    One (1) year experience - Preferred; 
    Certified Nurse Aide – Preferred
    Must maintain annual competency evaluation scores as determined by regulatory requirements.


Licensure, Certifications, and Clearances:

Preferred: Nurse Aide Registry in good standing either in the state where the facility is located or in a state covered by a licensure compact agreement with the state where the facility is located. 
Required: CPR required based on AHA standards that include both a didactic and skills demonstration component within 30 days of hire
