UPMC complies with all governmental requirements related to local, state, and federal COVID-19 vaccination for employment. The Jan. 13 Supreme Court of the United States decision that the Centers for Medicare & Medicaid Services federal COVID-19 vaccine mandate will move forward requires UPMC to ensure employees either get vaccinated or receive a requested medical or religious exemption.

Purpose:
The Professional Staff Nurse is responsible for the delivery of skilled nursing services to patients in their homes. If you’re a nurse who wants to provide one-on-one care to educate patients and family members, home health has an opportunity to provide individualized patient care. Home health allows flexibility in your day while providing hand-delivered care to our most vulnerable population.

Responsibilities:

  • Attend compliance training and adhere to the organization’s standards of conduct, policies, and procedures.
  • Focuses on customer service and continually strives to perform their job duties in a manner that will result in optimal patient satisfaction. Assigns other nursing personnel, including LPNs and Aides, to assist in care delivery.
  • Collaborates with a multi-disciplinary health care team to ensure positive patient outcomes. Performs duties and job responsibilities in a fashion that coincides with the service management philosophy of UPMC Home Health Care towards patients, visitors, staff, peers, physicians, and other departments within the organization.
  • Provides direct patient care, evaluates outcomes, and adjusts the nursing care process as indicated to ensure optimal patient care. Initiates, coordinates, and evaluates patient and family-centered health teaching in collaboration with the physician.

 

Qualifications

  • Graduate of an accredited school of professional nursing is required.
  • Bachelor's degree preferred.
  • UPMC Home Health Care approved certification preferred.
  • Minimum of 6 months of nursing experience in an Acute Care Facility or Skilled Care Facility.


Licensure, Certifications, and Clearances:
UPMC approved national certification preferred. Current licensure as a Registered Professional Nurse either in the state where the facility is located or in a state covered by a licensure compact agreement with the state where the facility is located. CPR is required based on AHA standards that include both a didactic and skills demonstration component within 30 days of hire
 

  • Basic Life Support (BLS) OR Cardiopulmonary Resuscitation (CPR)
  • Registered Nurse (RN)
