We are hiring for a Medical Case Manager at our Polyclinic Medical Center. The Medical Case Manager provides HIV medical case management per Pennsylvania Case Management Project guidelines for approximately fifty REACCH clients. Functions as a professional, participating member of the entire multidisciplinary REACCH Team, with the goal of lessening total viral load for all of the patients at REACCH. Uses therapeutic interactions and motivational interviewing to coach patients to meet goals. Under the oversight of the Program Coordinator and with accountability to the entire REACCH Team, functions within the clinic and within the community with moderate independence, and as a positive representation of REACCH and of the PinnacleHealth System.

  • Demonstrates flexibility to constantly reprioritize work based on acuity of patient need and based on the overall needs of the REACCH Team. Communicates professionally, both verbally and in writing (including in electronic communications). Promotes and maintains professional working relationships within the department, with clients, and within other areas of PinnacleHealth, the community, and outside agencies. Demonstrates a respect for individual dignity and diversity, both for our patients and for fellow employees. Demonstrates a strong commitment to patient care, and demonstrates an understanding of therapeutic boundaries with patients.   • Complies with all PinnacleHealth policies and procedures, especially regarding HIPAA and Pennsylvania HIV law.
  • Assists with general clinic set-up and clean-up when needed.
  • Answers phone calls within REACCH and directs calls to the appropriate Team Member.
  • Participates in chart audits and data collection for grant reports.
  • Initiates or participates in appropriate educational or outreach events for REACCH patient-clients or for the community.
  • Attends and participates in appropriate grant-related meetings or community events, such as human services partnership meetings, Family Health Council?s AIDS Coalition meeting, and/or community outreach events.
  • Maintains continuous education credits and attends trainings as required by the Pennsylvania Case Management Project guidelines.
  • Attends and participates in REACCH Team and other meetings, including Continuous Quality Improvement Meeting, Advisory Meeting, and Consumer Advisory Board.
  • Facilitates client retention in medical care through individualized case management, as required to meet goals set by the funding grants and the REACCH Program.
  • Maintains accurate documentation of services rendered, and documents on each patient within 1-3 days of the patient interaction. Enters appropriate data in CareWare, the electronic health record, and any other electronic or paper information systems required by the grants or the REACCH program.
  • Tracks case management on a care plan that aligns with Pennsylvania case management guidelines, and updates the care plan as required.
  • Uses motivational interviewing to determine the client?s goals, and therapeutically coaches client to meet goals. Encourages and educates each client to make healthy choices related to reducing viral load and increasing CD4 count. Provides accurate health education related to risk reduction.
  • Is responsible for coordination of medical and related services for case managed clients, such as transportation for appointments and help in obtaining durable medical equipment.
  • Assists clients in obtaining basic needs, including life sustaining medications, shelter, and food. Makes appropriate referrals as needed.
  • Regularly meets face-to-face and/or by phone with each case managed client, in a manner and on a schedule that is consistent with case management guidelines.
  • Performs initial intake and needs assessment for case managed clients. Ensures the client completes recertification paperwork and is eligible for the case management program.
  • Participates with REACCH Team to identify patients most in need of case management and presents case management as an option to those patients. Successfully recruits a caseload of approximately fifty patient-clients and maintains this caseload.



Bachelor's degree in social work or a related field required.Experience with Excel and data management systems preferred.2-3 years of experience in HIV medical case management is preferred.

Licensure, Certifications, and Clearances:
Clearances must be dated within 90 days
