Provide brief problem focused assessments, psychosocial support to include end of life and bereavement services, and timely and appropriate discharge planning based on anticipated needs.



  • Coordinate and facilitate Family Support Group and make available educational material for residents, families/sponsors and the general public.
  • Explain admission documents to residents/families.
  • Provide crisis intervention for unit emergencies.
  • Assist resident and family with adjustment to placement.
  • Assist residents/families in obtaining consults or appointments with internal and external services.
  • Assist resident and/or family in formulating resident goals. Document goals and Social Service approach to assist resident in meeting goals. Document progress and update care plan as required within defined timeframes outlined in departmental policies.
  • Assist resident or responsible party in completing the application for Medical Assistance.
  • Invite residents, who are capable of participating, and families to attend and participate in care planning/care conference meetings.
  • Represent Department at monthly Family Council meeting
  • Represent Department at monthly Resident Council meeting.
  • As assigned by RNAC, lead care planning conferences and complete necessary documentation within defined timeframes.
  • Listen to and counsels resident/family regarding issues related to death, dying, grieving, loss, change, disability, and assertive behavior.
  • Explain applicable long-term care regulations relating to Resident Rights, Medicare, and Medical Assistance to resident and/or family.
  • Observe all facility safety policies and procedures, Resident Rights policies and Infection control policies and procedures in the performance of assigned duties.
  • Participate in the resident lost and found program.
  • Maintain ongoing supportive contacts with resident and resident's family, documenting its effect on the residents.
  • Complete discharge summary for all assigned residents, including CTB, discharges, and transfer out as required, within timeframes outlined in policy.
  • Assist residents/families with room changes as necessary. Document discussions prior to room changes and documents resident's adjustment to changes in room within timeframes outlined in departmental policies.
  • Complete and document accurate initial and quarterly assessments, including physical, psychological, social and spiritual factors impacting on a resident within timeframes outlined in departmental policies.
  • Develop and maintain good relations with community agencies/individuals which may be resources for the Department.
  • Collect social history data within timeframes outlined in departmental policies.
  • Participate in the process for ensuring resident is provided with drawer key upon admission to facility and monitors compliance.
  • Proactively assist residents/families in resolving issues or concerns. When appropriate, coordinate meetings with members of the intra-disciplinary team to assist in problem resolution.
  • Monitor department to ensure individual compliance with defined timeframes of documentation. Develop action plans for staff not consistently meeting departmental standards.
  • Participate on facility committees as assigned.
  • Coordinate admission processes as directed, in absence of Admissions Director.
  • Document on the interdisciplinary care plan social service goals, anticipate discharge plan, and update as changes occur, within timeframes outlined in departmental policies.
  • Provide the public with information on SMSP, long term care, advance medical directives, and community resources.
  • Attend all assigned in-services.
  • Complete the Social Service portion of the MDS, and complete a corresponding progress note within timeframes outlined in departmental policies.
  • Carry pager to co-ordinate admissions after hours and on week-ends in rotation with other Social Workers and Admissions Director.
  • Coordinate discharge plan with facility staff, resident, family, and appropriate community agencies, and make timely and appropriate referrals. Discharge planning begins on admission. Document discharge planning progress in Social Service progress notes in the timeframes outlined in policy.



  • B.S. degree in Human Services required.
  • Experience in health care and/or geriatrics preferred.
  • Experience in long term care preferred.
