Coordinates scheduling functions for the multidisciplinary team under the direction of the Supervisor.



  • Monitors referral software package for new admissions and resumptions of care and notifies Scheduling Coordinator.
  • Calls all new patients for an admission visit or Resumption of Care visit.
  • Forward individual schedules to staff for patient visits next day.
  • Focuses on customer service and continually strives to perform duties of their job in a manner that will result in optimal patient satisfaction. Answers patient calls timely and forwards appropriately for resolution to promote patient satisfaction and increase our patient's perception of the timeliness of how the department dealt with issues and/or concerns.
  • Proven ability to facilitate change. Proven ability to multi task and prioritize work
  • Performs duties and job responsibilities in a fashion which coincides with the service management philosophy of the organization.
  • Reconciles/validates and processes daily activity records for all patient visits for individual staff.
  • Efficient use of Scheduling software package.
  • Identifies, develops and participates in process improvement opportunities within home health that will enhance the quality of service we provide.
  • Rotate weekends and holidays with flexible hours.
  • Schedules all patient visits based on patient needs, productivity expectations and staffing.
  • Updates the schedule daily as directed by the Case Manager and/or Scheduling Coordinator.
  • Schedule admissions within 24 hours of receipt but no later than 48 hours.
  • Attends compliance training and adheres to the organization standards of conduct, policies and procedures.



  • High School Diploma or GED required
  • Competency with PC Microsoft Office required
  • Administrative experience preferred
  • Medical Terminology Experience preferred

Licensure, Certifications, and Clearances:

  • Act 34
