Purpose:

Responsible for providing individualized care to homebound patients of all ages and diagnoses in their homes.  The Professional Staff Nurse follows physician orders and collaborates with a multi-disciplinary team to meet the needs of our clients.  Structured orientation and education provided for all staff.  Independent visits allow for flexible scheduling within your workday, and clinical oversight is always available. 

Responsibilities:

•             Provide direct patient care in the patient’s home setting

•             Thorough head to toe assessments for each patient

•             Complete treatment as ordered by a physician (ex:  wound care, IV therapy, pleurx catheter drainage)

•             Provide patients and families with education to help meet their goals of care

•             Communicate any change in condition to the physician and home care team timely

•             Work as part of a multidisciplinary team

•             Complete documentation same-day as visit

•             Follow-up with completed lab work, physician phone calls, or other concerns

•             Order supplies for patients timely and as appropriate

•             Request insurance authorization for visits as needed

•             Participate in mandatory meetings and complete scheduled education

 

 

Qualifications

  • Graduate of an accredited school of professional nursing is required.
  • Bachelor's degree preferred.UPMC approved certification preferred.
  • Minimum of 6 months of nursing experience in an Acute Care or Skilled Care Facility or Home Health or Hospice Care required.

Licensure, Certifications, and Clearances:
UPMC approved national certification preferred.

Current licensure as a Registered Professional Nurse either in the state where the facility is located or in a state covered by a licensure compact agreement with the state where the facility is located.

CPR is required based on AHA standards that include both a didactic and skills demonstration component within 30 days of hire
