Description

The Green Home, a Skilled Nursing and Rehabilitation facility located in Wellsboro, PA is seeking a Wound Care Nurse to join our team!  This position is Monday - Friday, one weekend a month and holiday rotation required.

The Wound Care Nurse (LPN) is responsible for the coordination and implementation of the facility's Skin Integrity and Wound Management Program including the promotion and review of standards of care and clinical competencies. This position is responsible for providing skin & wound care services, resident and staff education, including orientation and ongoing in-services to enhance the competency of the staff in order to meet the needs of the residents.  Under the direction of the DON and in collaboration with the nurses, Unit Managers, and other health care professionals, the Wound Care Nurse applies clinical knowledge and skills to address the resident's needs in order to achieve quality resident care outcomes.

Responsibilities:

  • Implements facility skin and wound care program to include resident care protocols, documentation tools, communication process for changes in skin conditions.
  • Promotes adherence to standards of care, competencies, policies and procedures in order to provide quality, effective wound care.
  • Provides basic wound care information and conveys facility wound management program elements and training to nursing staff.
  • Makes recommendations to the physician for changes to wound care orders, and provides evidence based research support as needed.
  • Serves as a role model for new hires and students.
  • Participates in programs/meetings as required and assigned: o Care Planning o Care Conference o Quality Improvement o Nursing/Facility management o Staff meetings
  • Ensures that residents and families receive the highest quality of care in a compassionate atmosphere that respects individual needs and rights. 
  • Demonstrates a commitment to the development and success of the Quality Improvement program and oversees the quality improvement activities as assigned.
  • Participates in SRC organizational committees and clinical practice councils and facilitates the implementation of new and revised practices, policies and programs Other duties as assigned.
  • Provides input on selection of staff and performance reviews.
  • Chairs facility wide committees as indicated or assigned.
  • Participates in organizational committees/practice councils, and facilitates the implementation/integration of new/revised practices, policies, programs and processes into facility operations.
  • Monitors resident wounds for appropriate treatment and healing progression. 
  • Actively participates in the development of resident's plan of care & treatment.
  • Utilizes nursing process in managing the care of residents including assessment, interdisciplinary planning, implementation, and evaluation of resident care, teaching and discharge planning.
  • Collaborates with physician and care team to ensure quality resident care and is accountable for the achievement of outcomes.
  • Anticipates/responds to emergent situations and initiates appropriate actions.
  • Documents in the medical record according to established procedures. 
  • Anticipates resident's potential risks by detecting subtle changes in resident condition/behavior and reports and records appropriately.
  • Collaborates with facility clinical leadership to identify education needs. Designs quality assurance and improvement activities and resolves problems related to team members performance, resident care, equipment and supplies. 

 

Qualifications

Completion of a Practical Nursing Program approved by the State Board of Nursing.  One year of experience as an LPN, preferably in a long term care setting. Solid/Strong/Good or higher rating on most recent performance evaluation for internal candidates.  Wound care experience preferred. Completion of Aging Institute's Geriatric Sensitivity and Certificate Program. Completion of UPMC SRL Mentoring Workshop.Act as a preceptor for new LPNs

Licensure, Certifications, and Clearances:
Board Certification preferred.  CPR required based on AHA standards that include both a didactic and skills demonstration component within 30 days of hire.  Currently licensed as Licensed Practical Nurse in Pennsylvania.  Must complete licensure examination within FOUR MONTHS of hire or within one year of graduation, whichever comes first.? Though temporary permits are valid for one year from the date of graduation, not one year from issue of permit (or until the results of the examination are known at which time it becomes null and void), UPMC requires that GNs take the licensure examination within 4 months of their start date or they will be terminated or demoted from the GN position, at the discretion of the BU.
