UPMC Innovative Homecare Solutions is hiring a Director, Palliative Care and APPs!


This position is responsible for clinical oversight of community based, care coordination and clinical programs for patients with serious illness, within UPMC's Health Services division. UPMC Innovative Homecare Solutions (IHS) is a mission-driven company that includes programs designed to understand and facilitate patient goals of care, consistent with Palliative Care domains. Specific clinical programs include palliative home care, Advance Illness Care, primary care home visits, chronic care management, and the Connected Care (CC) tele-home-monitoring program, with its call-center. This position requires the incumbent to work collaboratively with home-health teams, Health Plan care management, and medical direction from post-acute and palliative care, to integrate palliative care concepts and techniques into existing operational processes and to implement patient and family-directed educational strategies in home settings. The incumbent is responsible for ensuring the capture of appropriate performance metrics for these areas, and for facilitating patient-specific communication with primary care medical homes, and with payer-specific care management.

 

Responsibilities:

  • Serve as a liaison between IHS, the Health Plan, and physician practices.Works as a member of a team, among other leaders from UPMC?s Home Healthcare, and IHS Management teams. 
  • Supports collaborative program development with UPMC Homehealth Care, including Home Transitions, AIC, and Hospice.
  • Provides management of revenue cycle for the CRNP practice, both by maintaining Palliative Census and ensuring that budgeted Comprehensive Health Assessments are completed each calendar year.
  • Provides clinical support to the Director of Operations for AIC and CC programs.Provides input, support, and direction to Systems Analysts, including for application used for Connected Care, primary care documentation, and Home Health care.
  • Provide oversight of the community-based practice for Nurse Practitioners (UPMC APP), specifically related to programs of home palliative care, Advance Illness, and primary-care medical home visits.Provide support and direction to the Care Transitions, Advanced Illness Program and CC Call Center.
  • Provide on-going collaborative interaction with the Vice President of Compliance and Quality and the Medical Director related to CC, AIC, HT programsProvide on-going feedback and seeks consultation and guidance for program development and oversight from the Medical Director for IHS.
  • Provide clinical oversight of the Advanced Illness Care (AIC) Program clinical staff Provide clinical oversight of the Comprehensive Health Assessment program.
  • Provide Clinical direction to the CC Call Center and associated Tele monitoring programs
  • Coordinates program implementation and maintenance of services provided under AIC and CC programs to non-UPMC organizations where appropriate and approved by Director of Operations and IHS Executive leadership.
  • Be clinically active for a minimum of 25% Comprehensive Health Assessments and Palliative Care visits, be competent in telemedicine using both My UPMC and Anywhere Care as a platform.

 

Qualifications

  • Completion of an accredited school of professional nursing Bachelor's degree (BSN) required.
  • Master's degree in related field preferred.
  • 5 years healthcare experience required.
  • 3 years management experience required.
  • 3 years experience in palliative care.
  • 3 years home health experience preferred
  • Certification in performance improvement/ project management preferred.
  • Strong communication skills Ability to work in complex environments.



Licensure, Certifications, and Clearances:
Certified Registered Nurse Practitioner or Advance Practice Nurse, Automotive Insurance, Driver's License
