Our team at UPMC Senior Communities is looking for a Full-time Clinical Nurse Liaison!!
In this role, you would assess and evaluate patients clinically for appropriateness for admission to skilled nursing facilities. To promote the continuum of post-acute services provided throughout the UPMC Health System. The Clinical Liaison promotes the achievement of facility census requirements through direct contacts and appropriate referral sources.
Location: 200 Lothrop St. Forbes Tower, Pittsburgh PA 15219 (must be willing to travel to multiple facilities if needed) (travel costs may be expensed)
Monday-Friday, 8-4:30pm, no weekends/holidays
Responsibilities:
- Provides assistance, ideas, and input to Management to improve the overall effectiveness of the facilities.
- Develops and maintains a thorough understanding of inpatient and outpatient treatment programs, modalities, philosophies, goals, and objectives. Understands and is able to communicate admission criteria for each program.
- Provides management with full details of process improvement needs on nursing assessment and patient care issues.
- Provides resource and education services about UPMC Skilled Nursing Facilities services and admission standards to patients, families, care management teams, and physicians.
- Provide informative literature about UPMC Skilled Nursing Facilities to internal referral sources, Establish and maintain education stations and locations. Place and distributes brochures, posters, and other materials as needed.
- Applies nursing skills to promote and ensure customer satisfaction. Analyzes and solves customer problems by preparing and providing solutions to problems, and follows up on referrals to ensure customer satisfaction and promote positive outcomes.
- Complete appropriate documentation and paperwork accurately and within the acceptable time frame.
- Conduct on-site assessments and establish priorities in the allotted time frame to the appropriate intake person, maintaining accuracy, attention to detail, while maintaining confidentiality and privacy in compliance with HIPPA.
- Works with internal Case Management and Facility Administration to jointly handle customer complaints; patient care issues, problems, and concerns and takes corrective action when necessary.
- Provides management with Performance Improvement information such as monthly referrals, referral sources, admissions, conversion of referrals, and disposition of non-converted referrals.
- Assist in pre-admission care planning. Identifying and recommending special equipment needs and precautions to assure a safe continuum of care and making recommendations regarding cost-effective and appropriate admission orders.
- Makes sales and census development contacts while in the hospital and with other referral sources.
- Develops and maintains a thorough understanding of the state of the art nursing standards of practice.
- Develops and maintains knowledge of the skilled nursing facilities and services of Senior Communities.
- Maintains high moral and business ethics in carrying out responsibilities and duties
- Performs all duties and responsibilities according to the philosophy and standards of the UPMC Health System and Senior Communities embracing the system-wide core competencies which include: A) Making quality patient care and patient satisfaction the number one priority B) Initialing and participating in performance improvement initiatives. C) Conveying courtesy, respect enthusiasm, and positive attitudes with patients, staff, medical staff, and visitors.D) Maintaining confidentiality at all times.
- Provides management with relevant information and offers input regarding external developments and opportunities.
- Review patient records and obtain any relevant patient information, assessing for patient appropriateness and medical stability to facilitate a timely admission and transfer to an appropriate UPMC skilled facility. Works with resources to identify appropriate referral placement.
Qualifications
Required:
- BSN and/or BS in the health-related field required.
- The candidate must possess a current license.
- The candidate should have at least 5 years of previous experience in acute care and long-term care settings or equivalent experience.
Preferred:
- 2 years of marketing experience in a health care setting is preferred.
- 2 years of skilled nursing experience or previous skilled nursing liaison experience with basic regulatory knowledge preferred.
Licensure, Certifications, and Clearances:
CPR is required based on AHA standards that include both a didactic and skills demonstration component within 30 days of hire
- Basic Life Support (BLS) OR Cardiopulmonary Resuscitation (CPR)
- Registered Nurse (RN)
- Act 34
- OAPSA
