UPMC complies with all governmental requirements related to local, state, and federal COVID-19 vaccination for employment. The Jan. 13 Supreme Court of the United States decision that the Centers for Medicare & Medicaid Services federal COVID-19 vaccine mandate will move forward requires UPMC to ensure employees either get vaccinated or receive a requested medical or religious exemption.

  • Gain an extensive knowledge of long-term home and community based services, including each of the UPMC Community Provider Service lines to ensure that referrals are appropriately made to meet the client's needs.
  • Complete documentation of phone calls and personal visits in a timely manner as per the department's policy.
  • Routinely provide critical information with regards to the client to the Care Coordinator (Community Worker) to ensure needs are addressed proactively.
  • Provide medication management to the clients to include medication reconciliation, medication administration, assist the client with medication refills and communicate with the client and attending regarding recommendations on medication appropriateness.
  • Engage the client/family member to understand and adhere to the plan of care.
  • Participate as a speaker on various health topics to community groups, and other health professionals as needed.
  • Participate in all department staff meetings.
  • Interact with client's physician routinely to communicate critical information and seek direction with regards to medication management and other plan of care.
  • Serve as a preceptor for nursing students, pharmacy students, and medical residents as part of their training in community health.
  • Participate in inter-disciplinary care meetings as needed.
  • Perform client assessments and reassessments for status changes and develop care plans based on sound geriatric principles to meet the needs of the client.
  • Identify possible unmet health care, social and psycho-social needs and appropriately refer those clients to available resources within the department (social workers), UPMC or community agencies.
  • Assist the client to maintain complete personal health information such as a health care diary and assist them to effectively communicate their health care status with their physicians or other health care providers.
  • Perform blood pressure, cholesterol, and glucose screenings as requested in the community (i.e. senior centers, senior hi-rises, etc.).
  • Participate in educational activities in order to gain new skills and update current knowledge to include Certificate Program in Gerontology offered by the University of Pittsburgh Institute on Aging and Board Certification in Gerontology offered by ANCC.
  • Participate in health fairs, health screenings, vaccination programs etc., as requested.
  • Actively participate in the program's continuous quality improvement program and in research endeavors.



  • Completion of an accredited school of nursing required.
  • BSN preferred.
  • 3 years experience in gerontological or medical/surgical nursing required.
  • 2 years in home care setting is preferred.
  • Excellent nursing assessment skills.
  • Excellent written and verbal communication skills.
  • Working knowledge of the commonly used computer systems for data and word processing systems and the ability to use internet.
  • Ability to function independently and prioritize assignments appropriately.
  • Valid PA driver's license and have access to an automobile.

Licensure, Certifications, and Clearances:
CPR required based on AHA standards that include both a didactic and skills demonstration component within 30 days of hire

  • Basic Life Support (BLS) OR Cardiopulmonary Resuscitation (CPR)
  • Registered Nurse (RN)
  • Act 34
