UPMC Chartwell is looking for a full time Infusion Nurse to join their dedicated team in Altoona, PA.
The Infusion Nurse coordinates and performs skilled nursing care for patients in the home setting. Participates in teaching patients and significant others home medical management; and provides leadership by working cooperatively with staff to establish priorities and goals of patient care.
This role will work 9 am to 5:30 pm, Monday through Friday and rotating on call weekend shifts.

Responsibilities:
- Attend scheduled patient care conferences, in-services, departmental meetings, etc. Maintain professional licensure, certification and health requirements as well as demonstrating clinical competence of orientation programs, etc. Participate in the orientation of other nurses or center personnel, as requested. Support the marketing efforts of the center by assisting the sales team and providing educational programs to referral sources, as requested.
- Take professional responsibility for self-protection during required work in potentially hazardous situations (i.e., handling of cytoxic agents, handling of blood and body fluids). Represent self in a professional and ethical; manner at all times. Comply with all applicable company policies and procedures. Participate in the activities of the Performance Improvement Program.
- Participate in Chartwell Pennsylvania's Performance Improvement Program. Promote Chartwell culture by providing quality, comprehensive services to all customers through a team approach.Demonstrates ability to communicate effectively and express ideas clearly.
- Demonstrate reliability and follow-through on all assigned tasks. Complete work on schedule. Demonstrate ability to work effectively and maintain expected productivity.
- Communicate with others in a warm and helpful manner while simultaneously building credibility and rapport. Maintain courteous demeanor at all times. Take initiative to present ideas/ suggestion to management. Utilize appropriate communication lines in relaying problems, concerns, questions, and ideas. Set high goals or standards of performance for self. Make sure that work meets quality standards. Show pride in work.
- Provide infusion and other skilled nursing care to patients. Provide pre-discharge patient assessments when appropriate. Complete all required patient care documentation and reports in a timely manner. Submit all required patient care documentation and reports in a timely manner. Communicate to supervisor, physicians and other health care providers, as appropriate, any changes in the patient's condition.
- Take on call responsibility as assigned to provide 24 hour availability, and provide these services in accordance with established protocols. Maintain confidentiality of patient information. Display a neat, clean, professional appearance at all times.
Qualifications
- Diploma from an accredited school of nursing required.
- B.S.N. preferred.
- 1 year previous specialized home health experience preferred.
- 1 year clinical experience involving infusion therapy, including patient assessment skills, and venous access skills. CPR certification required.
- PICC training preferred.
- Excellent interpersonal, communication and organizational skills required.
- Ability to assume a flexible work schedule.
Licensure, Certifications, and Clearances:
CPR required based on AHA standards that include both a didactic and skills demonstration component within 30 days of hire
