Purpose:
Provide designated care under the direction of the primary service provider in the home in order to achieve identified goals. Function as an extension of skilled nursing services and/or rehabilitation services in the home.
Responsibilities:
- Communicates appropriately with the interdisciplinary home care team in a timely manner, reporting any changes in patient?s condition to appropriate professional staff.
- Attends compliance training and adheres to the organization standards of conduct, policies and procedures.
- Focuses on customer service and continually strives to perform the duties of their job in a manner that will result in optimal patient satisfaction.
- Provides safe and direct personal care to patients in the home setting under direct supervision of appropriate professional.
- Performs duties and job responsibilities in a fashion which coincides with the service management philosophy of the organization towards patients, visitors, staff, peers, physicians and other departments within the organization.
- Identifies, develops and participates in process improvement opportunities within the home health agency that will enhance the quality of services we provide.
- Proven ability to facilitate change.
- Maintains and completes documentation accurately and promptly while in the patient's home and within the agency and regulatory standards.
- Participates in all required agency meetings.
Qualifications
- High School Diploma or GED - Required
- Required: successfully completed a training and competency evaluation program as specified in Home Health and/or Hospice as applicable: Condition of participation: Home Health aide services.
- One (1) year experience - Preferred
- Certified Nurse Aide Preferred
- Must maintain annual competency evaluation scores as determined by regulatory requirements.
Licensure, Certifications, and Clearances:
Preferred: Nurse Aide Registry in good standing either in the state where the facility is located or in a state covered by a licensure compact agreement with the state where the facility is located. Required: CPR required based on AHA standards that include both a didactic and skills demonstration component within 30 days of hire
- Automotive Insurance
- Basic Life Support (BLS) OR Cardiopulmonary Resuscitation (CPR)
- Driver's License
- Act 33 with renewal
