Community HealthChoices (CHC) is Pennsylvania's managed care long-term services and supports (LTSS) program serving seniors and individuals with physical disabilities in the Commonwealth who are covered by Medicare and Medicaid.
This position is a hybrid role and will require coming into the office a couple of days a week.
The LTSS Manager is responsible for the operational functions of a LTSS team including the direct supervision, coaching, and counseling of staff, manage, supervise, and coordinate the LTSS operations of the CHC Team. The Manager is responsible for the day-to-day service coordination functions through the management of staff and activity which may include behavioral health, utilization management, and care management. Serves as a liaison to government and other regulatory agencies, and internal departments. Monitors and collects information related to staff and programs, performance review against goals, and makes recommendations for improvement. Supports supervisors managing service coordination and care management staff.
Responsibilities:
- Participates in service coordination rounds, members ICT, and program development.
- Selects, schedules, assigns, and evaluates LTSS staff, adjusting hours as necessary.
- Participates in the development and modifications of CHC LTSS programs.
- Investigates and resolves complaints from members and providers.
- Interprets and enforces department and organization policies.
- Participates in appropriate department and organization committees.
- Develops, implements, and evaluates LTSS policies, goals, and objectives.
- Recruits, trains, and orients new staff and assures that training and education needs of staff are met.
- Directs the service coordination process for all LTSS staff.
Qualifications
Bachelor's degree (B. A.) from a four-year College or University. Five to seven years of related experience and/or training. Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations. Ability to write reports, business correspondence, and procedure manuals. Ability to effectively present information and respond to questions from groups of managers, clients, customers, and the general public. - Strong Presentation skills preferred
Licensure, Certifications, and Clearances:
RN or LPN, LCPC, LPC, LCSW preferred
