Description

Purpose:
Ensure the provision of cost-effective pharmacotherapy contributing to optimal patient outcomes, provide experiential education for present and future health care practitioners, and support drug-related research.

Responsibilities:

 

  • Collaborating with Primary Care and Specialty practices, the Ambulatory Clinical Pharmacist will design, evaluate and modify pharmacotherapy for specific populations in ambulatory care.
  • Implements protocols to enhance pharmaceutical care services for patients served by the Medical Group practices.
  • Strives to create meaningful, interactive relationships within the patient-centered medical home and help to achieve high quality health outcomes for populations served.
  • This role will support quality improvement and resource management initiatives, while participating in scholarly activities and/or clinical research in ambulatory care.
  • Opportunity to educate health professionals and patients regarding optimal medication therapy.
  • Complete Medication Therapy Management sessions one-on-one with patient populations, either through telephonic on in-person sessions. 

Qualifications

  • Requirements:
    • Pharm.D required. B.S. Pharmacy Required.
    • Pennsylvania state license required.
    • Must have excellent interpersonal, verbal, and written communication skills.
    • Very high level of independent judgment and problem-solving skills.
    • Strong attention to detail.
    • Record of effective working relationships with other health professionals.
    • Excellent oral and written communication skills.
    • Strong analytical, prioritization, project management and organizational skills.
    • Ability to work within a changing environment and drive results.
    • Demonstrate success in relationship building.
  • Preference: 3-5 years ambulatory pharmacy experience or completion of PGY2 ambulatory care residency.


Licensure, Certifications, and Clearances:
Clearances must be dated within 90 days
