Description

Community HealthChoices (CHC) is Pennsylvania's managed care long-term services and supports (LTSS) program serving seniors and individuals with physical disabilities in the Commonwealth who are covered by Medicare and Medicaid. 

The Community Health Promotion Specialist will identify key community and provider partners who support UPMC Health Plan goals and objectives. This position assesses the scope of existing community-based projects, identifies community needs, and identifies potential collaborative initiatives. Facilitates communication among organizations. Collaborates with other internal departments in the development and implementation of programs that have a community focus. Participates on community or other agency boards to further Health Plan goals and objectives. The Community Health Promotion Specialist works with health-related subject matter experts on the design, development, and delivery of health awareness information. Collaborates with other internal staff on the development and implementation of health awareness programs. Organizes and participates in community health promotion events.

Responsibilities:

  • Identifies other UPMC resources, community and provider partners who have the ability to support UPMC Health Plan's goals for improving service and the quality of care provided to the membership.
  • Completes in a timely manner required Department of Public Welfare, Pennsylvania Insurance Department or other governmental or regulatory reports on community involvement or events.
  • Develops project plans and monitors status of assigned projects. Provides written summaries to supervisor of status on a frequent basis. Provides support to initiatives as requested.
  • Assists in the development of training materials related to community relations. Participates in education programs related to scope of community relations department for internal staff and external groups.
  • Assists supervisor in the development of community based initiatives which support the goals and objectives of UPMC Health Plan.
  • Facilitates collaboration between external health resources and UPMC Health Plan.
  • Collaborates with internal departments in the development of plans involving community and provider partners to improve service and the quality of care provided to the membership. Serves as a resource to internal departments.
  • Identifies community leaders, organizations and providers who provide services to consumers who may be eligible for UPMC Health Plan products. Provides information on UPMC products and links to appropriate Health Plan departments.
  • Perform duties and responsibilities in accordance with the philosophy and standards of the UPMC Health Plan, including conveying courtesy, respect, enthusiasm, and a positive attitude through contacts with staff, health plan members, peers and external contacts.
  • Plans, coordinates and implements community health promotion events. Adheres to Department of Public Welfare, Pennsylvania Insurance Department or other governmental or regulatory requirements for participation in community events or incentives. Obtains required approvals prior to events. Maintains documentation. Develops and monitors budgets for events. Collaborates with other UPMC and UPMC Health Plan staff as appropriate. Attends events. Provides health education materials and information on UPMC Health Plan products to interested consumers. Provides written summaries of outcomes of community events.
  • Assist supervisor in the research, review and interpretation of health plan clinical outcomes and other valid published literature on health disparity, health outcomes and demographic data of the communities served by UPMC Health Plan. Assist supervisor in identifying the areas where opportunities for improvement exist.

 

Qualifications

Bachelor's degree in a health-related discipline or in social services, communications, etc., and 2-3 years experience working in a health care or community-based organization required OR high school and 6 years experience working in a health care or community-based organization required. Experience working in a managed care environment preferred. Knowledge of community-based programs within UPMC Health Plan's service area is preferred. Ability to plan, implement and evaluate community health promotion events required. Working knowledge of health behavior change and motivational strategies preferred. Proven administrative abilities, excellent interpersonal skills, and the ability to work in a team environment with a minimum of supervision required. A high energy level and excellent written and oral communication skills are required. Ability to travel required. Intermediate-level proficiency with Microsoft Office products including Word, Excel, and PowerPoint required.
