UPMC complies with all governmental requirements related to local, state, and federal COVID-19 vaccination for employment. The Jan. 13 Supreme Court of the United States decision that the Centers for Medicare & Medicaid Services federal COVID-19 vaccine mandate will move forward requires UPMC to ensure employees either get vaccinated or receive a requested medical or religious exemption.

Clinical Transplant Coord I

Description

 Joining the transplant group at UPMC Presbyterian is an opportunity to experience new levels in your career! Our amazing team of nurses is specially trained to provide transplant patients and living donors with the best possible care! 

Purpose:

 

  • TRANSPLANT/VAD REFERRAL AND EVALUATION
  1.  Identifies appropriate transplant/VAD candidates upon referral and interacts with appropriate hospital staff.
  2.  Identifies requirements for candidate evaluation.
  3. Coordinates the evaluation process for potential transplant/VAD candidates with the multidisciplinary team
  4. Functions as candidate advocate by applying knowledge of growth, development, educational and cultural background during candidate and family interaction.
  5. Provides data to regulatory agencies.
  6. Demonstrates knowledge of compliance with United Network for Organ Sharing (UNOS) policies and listing requirements.
  7. This Clinical Transplant/VAD Coordinator will be On-Call Monday - Friday during evening hours.
  • POST-TRANSPLANT/VAD OUT-PATIENT PERIOD
  1.  Identifies and responds to recipient and family needs in post-transplant/VAD phase.
  2. Reinforces educational and follow-up requirements.
  3. Collects and reviews data pertinent to recipient's health and organ function.
  4. Provides and maintains comprehensive documentation of recipient's progress.
  5. Implements changes in drug therapy as ordered by physician.
  6. Arranges for consultations, diagnostic procedures, and hospitalization when indicated.
  7. Maintains communication with referring physician and other health care providers.
  8. Assesses recipient's compliance with medications and medical regime; collaborates with transplant/VAD team to attain and maintain compliance.
  • POST-TRANSPLANT/VAD IN-PATIENT PERIOD
  1.  Monitors patient progress post-transplant.
  2.  Educates recipient and family regarding care and responsibilities after transplantation.
  3. Collaborates with transplant/VAD team to establish and implement patient plan of care.
  • PROFESSIONAL GROWTH AND DEVELOPMENT
  1.  Obtains ABTC Certification.
  2. Attends practice-related conferences.
  3. Participates in educational and outreach activities.
  4. Constructively receives feedback and direction.
  5. Identifies and communicates learning needs to manager.
  6.  Takes action to improve knowledge, skills and performance based on feedback or on self-identified developmental needs.
  7.  Participates in self-review as requested by manager.
  8. Responsible for adhering to On-Call schedule.
  • PERI-OPERATIVE PERIOD
  1.  Participates in transplant/VAD process according to UPMC protocol.
  2. Reviews potential recipient's current medical status to determine eligibility for transplant.
  3. Facilitates admission or transfer to UPMC.
  4.  Communicates with organ procurement agency/ORC and collects the appropriate data necessary for decision making re organ acceptance/rejection for transplant as applicable.
  5. Exhibits ability to remove transplant recipient from the list per UNOS regulations as applicable.
  • PRE-TRANSPLANT/VAD PERIOD
  1.  Develops and institutes a teaching plan addressing transplant/VAD candidates learning needs.
  2.  Maintains communication with patient and referring physician.
  3.  Recognizes potential problems and/or significant changes during the waiting period and coordinates required care.
  4. Acts as a resource person for internal and external health care providers.
  5. Provides and maintains comprehensive documentation of candidate process.
  6. Exhibits ability to maintain and update candidate listing status per UNOS regulations as applicable.

 

 

Qualifications

3-5 years clinical nursing experience, and 2-3 years recent (within 5 years of hire date) clinical transplant/VAD coordinator experience required. Demonstrated competence in professional transplant/VAD nursing practice.

  • Diploma or Associates Degree in Nursing.
  • BSN preferred.
  • Must obtain a Solid/Strong/Good or higher on annual performance review to maintain Clinical Transplant Coordinator II status. Must obtain a Solid/Strong/Good or higher on performance review to be considered for promotion to Supervisor, Clinical Transplant Coordinator.


Licensure, Certifications, and Clearances:
Current Pennsylvania licensure as a Registered Professional Nurse. Certification from the American Board for Transplant Certification (ABTC) as a Certified Clinical Transplant Coordinator (CCTC), CHFN (Certified Heart Failure Nurse), or other UPMC approved certification associated with the service line required at time of hire.
