UPMC complies with all governmental requirements related to local, state, and federal COVID-19 vaccination for employment. The Jan. 13 Supreme Court of the United States decision that the Centers for Medicare & Medicaid Services federal COVID-19 vaccine mandate will move forward requires UPMC to ensure employees either get vaccinated or receive a requested medical or religious exemption.

Community Paramedic/EMT (Full-time)

Description

Do you have an active or previous Emergency Medical Technician (EMT) or Paramedic certification and at least one year of experience in a health-related field? Do you have a high school diploma or equivalent? If so, UPMC may have the perfect fit for you! 
 
UPMC Health Plan is hiring a full-time Community Paramedic to support the Connect Community Paramedics department. This is a Monday through Friday daylight role and will be based out of the Pittsburgh (Northside - Harbor Gardens), PA office location. Some travel will be required.

The Community Paramedic is responsible for staffing the daily operations of the CONNECT- Community Paramedic Program. They will be responsible for connecting vulnerable patients to available social determinate of health resources that could improve patients lives while promoting healthy steps, disease management skills, care transitions, and other services.

Responsibilities:

 

  • Interact with EMS personnel, ED staff and Care Management teams as necessary to coordinate care for patients.
  • Contact and help patients enroll in various social service programs as necessary to help patients close gaps in care.
  • Document and report all findings as requested.
  • Schedule and visit patients to complete social ecological assessments and follow up of patients as assigned.
  • Complete all training required in the timeframes established.

 

Qualifications

  • Highschool diploma or equivalent required.
  • Current licensure + Minimum of 1-year experience in public health, EMS, health screenings or other health-related fields
  • Proficient in MS Word/Excel
  • Ability to interact effectively with senior management, EMTs/paramedics, and program participants
  • Reliable transportation
  • Ability to work early morning hours and weekend hours as requested.
  • Prefer candidates currently working or living in CONNECT communities and/or Allegheny County.


Licensure, Certifications, and Clearances:

  • Emergency Medical Technician (EMT) OR Paramedic certification required 
  • Drivers' license required
  • Automotive Insurance
