Description
Purpose:
The Care Management Assistant provides clerical assistance with audits, patient interviews, and department assistance with the daily functions of the hospital case management department. This position does not provide direct patient care but does interact with patients and their families
Responsibilities:
- Perform chart reviews using the electronic medical record and collects data for ongoing compliance audits, report information in a clear, timely, and concise manner, and alert care management leadership of concerns.
- Reviews and completes forms as needed such as Important Message from Medicare, and freedom of choice with the patient and the patient’s support members.
- Assist care management team with resolving plan of care and discharge issues, escalates to care management leadership as appropriate
- Assist care management team with questions and troubleshooting issues that arise.
- Maintains strict confidentiality in compliance with HIPPA
- Performs all the responsibilities of the Administrative Assistant as needed: routine clerical functions such as copying, sorting, filing, and typing.
- Sets up and maintains varied office files, logs, and records.
- Orders office supplies and maintains office equipment.
- Answers, screens, and routes incoming calls and messages.
- Greets screens and directs visitors, vendors, and providers to appropriate care management staff.
- Comply with requirements of appropriate regulatory agencies.
- Performs all responsibilities in accordance with department and system-wide competencies/behaviors and values
- Performs admission interviews with patients to verify demographics, and pre-hospital living arrangements working closely with assigned care manager RN/SW to ensure a plan of care is started within 24 hours of patient admission.
- Assists patient/HUCs/care management staff with scheduling appointments for patients for primary care physician, ride home, or other as needed prior to discharge.
- Serves as a contact between the patients, care managers, and nursing staff
Qualifications
- High school diploma or equivalent required.
- If no previous hospital or pre-hospital training successful completion of UPMC nursing assistant training program or nursing student that has successfully completed one clinical rotation.
- CNA preferred
- Basic computer skills required (email, Microsoft Word/ PowerPoint, and calendar)
- Knowledge of medical terminology.
- Ability to effectively communicate both orally and in writing to patients and the care team
