Summary:
UPMC is hiring a Nurse Coordinator for the Center for Perioperative Care (CPC) working Monday - Friday daylight shifts located in Oakland, PA.  Potential for remote work in the future for this role.  You will provide care management and pre-surgical follow-up for all patients. Acts a “surgical coach” to help optimize patient outcomes Great collaborative team environment with opportunities for growth as we grow and expand the program. The Professional Nurse Coordinator is a Registered Nurse and is a member of the care delivery team.   

Responsibilities:

  • Demonstrates accountability for professional development that improves the quality of professional practice and the quality of patient care. Actively participates in practice-based shared governance, goal setting and supports the change and transition process to improve quality of care and the practice environment. Serves as a highly engaged and full partner on the care team and responds willingly to care team member needs for assistance and partnership. Participates in work that improves patient care and the professional practice environment. Adapts to change and demonstrates flexibility with the change process. Provides oversight and supervision to the overall practice environment, assisting all other disciplines and directing clinical/administrative activity as need arises. Demonstrates leadership capabilities with new nursing staff, patient information coordinators, medical assistants, and office assistant staff in the daily management of the patient process.
  • Provide Care management between disciplines and across other CPC sites 
  • Help optimize workflows in clinic
  • Partner with physicians, APPs, and other staff to coordinate care for patients seen in clinic
  • Manage eConsults process to ensure patients are scheduled and needs are met

 

Qualifications

  • Three (3) years nursing experience
  • BSN preferred.
  •  Epic, EKG experience, Care management/coordination preferred 


Licensure, Certifications, and Clearances:

  • Advanced Cardiac Life Support (ACLS) OR Basic Life Support (BLS) OR Cardiopulmonary Resuscitation (CPR)
  • Registered Nurse (RN)
  • Act 33 with renewal
  • Act 34 with renewal
  • Act 73 FBI Clearance with renewal
