UPMC is currently hiring an Outreach Social Worker - MSW to support the City of Pittsburgh neighborhoods.

This is a Monday-Friday Daylight position (7:30AM-4PM).


Under the general direction of the Director of Geriatric Care Coordination, the incumbent will act as a clinical liaison in the community for the Living-at-Home (LAH) Program and other services provided by UPMC Community Provider Services for older adults. He or she will seek out opportunities to provide both short-term and long-term clinical outreach services to organizations affiliated with the UPMC and the home care network in order to enhance the quality of health and wellness to its clients. Demonstrate expertise in clinical social work practice relative to geriatrics. Assist in the planning and further development of the program.



  • Carry a small LAH social work caseload when needed by the department.Assist the director with recruitment of clients and volunteers for the program.
  • Assist the director with proposal development for seeking funds, meeting with legislators and donors to seek funds and other fundraising activities.Participate in all department staff meetings.
  • Participate in Quality Improvement studies as well as staff development activities.Participate in department staff development program, UPMC sponsored conferences and workshops and profession in-service programs related to areas of direct clinical practice in order to maintain professional standards and social work licensure and to develop advanced clinical expertise relative to various treatment modalities.Maintain communication with Program nurses and social workers regarding clients.
  • Develop and implement an outreach program to promote geriatric care coordination offered by LAH in the senior hi-rises and senior centers within the LAH service area.As part of outreach, provide health education through health and wellness programming.
  • Present (or arrange for others to present) on various health topics lectures, workshops, etc. to the organizations and community groups identified as part of the outreach activities.Have the ability and necessary skills to assess clients and to identify unmet needs. Refer these clients to LAH, geriatric programs within UPMC, UPMC Community Provider Services and other services for older adults within the community.
  • Develop reports analyzing appropriate indicators of effectiveness of education/outreach efforts and compile these statistics on an ongoing basis.Provide service coordination at a senior hi-rise when needed through PHFA contract.
  • Under the general direction of the program director see out opportunities to promote services provided by UPMC and UPMC Community Provider Services. These outreach activities may include participation in health fairs, health screenings, educational events in senior centers, hi-rises, community groups, etc.Gain an extensive knowledge of the clinical services provided by UPMC and UPMC Community Provider Service lines serving older adults in order to promote the services appropriately.



  • Master of Social Work (MSW) degree from a CSWE accredited school of Social Work required
  • Current/active social work license (LSW) required or successful passing of social work licensing exam within 9 months of employment
  • Employees must sit for license exam within the first 4 months of hire.
