This full-time position performs a vital role by delivering high-quality HIV Navigation Services (HNS) to individuals at high risk of acquiring HIV. This role may accompany clients to their medical appointments at the REACCH Program, and/or other community agencies.  This individual must have excellent attention to detail skills and be able to fully assess situations and prioritize follow-up. The expected caseload for this position is up to 60 clients for HNS prevention services. Please note, this staff member must have the ability to work afternoon and evening hours at the REACCH Program 1 or 2 evenings per week (until 7:00 PM). This role will include performing rapid HIV testing, collection of STI tests, and linking individuals to the REACCH Program’s pre-exposure prophylaxis (PrEP) and post-exposure prophylaxis (PEP) services. Ample training and shadowing will be provided by the REACCH Program, the Pennsylvania Department of Health, and the program’s fiscal intermediary.  This position will report to the REACCH Program’s Case Management and Social Work Supervisor. Supervision will be provided for a minimum of one hour biweekly.


Primary Duties – HIV Services Navigation (HNS) and HIV Testing:

  • Provide HIV Navigation Services to 60 clients in the form of pre-exposure prophylaxis (PrEP) and post-exposure prophylaxis (PEP) referral and linkage, and other risk-reduction activities.
  • Prompt follow-up with clients, emergency room department, and referral agencies to ensure clients have access to PrEP or PEP medications and encourage adherence.
  • Serve as a resource to clients for community resources and referrals.
  • Schedule, collaborate and provide community outreach events for HIV/PrEP education and HIV testing with community partners and REACCH or GLO – Harrisburg team members.
  • Track and analyze HIV testing efforts to ensure the dept. is reaching high-impact populations.
  • Provide rapid HIV and STI testing both off-site in the community and on-site at the REACCH Program on at least a monthly basis.
  • Utilize EPIC, AFTIS, CareWare, EvalWeb, and NEDSS for data entry and reporting for prevention.

Secondary Duties:

  • Complete outreach to lost to care clients in emergency departments, client homes, and the community, as needed.
  • Cover acute support service needs for people living with HIV as assigned.
  • Complete other tasks as assigned.



  • Bachelor’s degree in related health or human service field required.
  • Demonstrated experience exhibiting exceptional organizational, assessment, teamwork, and critical thinking skills preferred.
  • Demonstrated experience and ability to work with people with diverse and marginalized backgrounds, such as LGBTQ+ and/or BIPOC clients strongly preferred.
  • Experience providing case management and knowledgeable about community resources in Dauphin county preferred.
  • An ability to demonstrate one’s commitment to social justice preferred.
  • Bilingual preferred.
  • Experience in the HIV/AIDS field or medical, hospital/community health settings preferred.
  • Experience discussing sensitive topics with clients such as sexual health, sexual assault, intimate partner violence,  etc.
  • Understanding of and the ability to apply motivational interviewing, solution-focused, trauma-informed care and strengths-based client-focused techniques preferred.

Licensure, Certifications, and Clearances:

Clearances must be dated within 90 days

  • Act 33 with renewal
  • Act 34 with renewal
  • Act 73 FBI Clearance with renewal
