Description
UPMC Transition of Care department is looking for a Licensed Practical Nurse (LPN) to join their team of 11 nurses. The LPN MUST work for the first 6 months out of the Lemoyne office, then will have the option to work remotely from home.
Hours: NO WEEKENDS! NO HOLIDAYS! Monday - Friday, FLEXIBLE betwwen the hours of 8am-6pm. You set your 8 hour shift & schedule between these hours.
Responsibilities:
- Receive inbound calls/ make outbound calls to follow up on patient's discharged from UPMC; check in on symptoms, make transition of care appointments with primary care physicians, medication reconciliation
- Daily list review for patients who meet criteria for transition of care calls
- Obtain chronic medication refills for discharged patients
- Create/update vaccine profiles
- Help with Emergency Department discharge follow-up calls
- Communicate with physician offices about patient symptoms, schedule PCP appointments
- Comply with requirements of appropriate regulatory agencies (i.e. OSHA, CLIA, PA Department of Health).
- Comply with all policies and procedures.
- Document all patient/family responses in the medical record.
- Maintain strict confidentiality related to medical records and other data.
Qualifications
- Graduate of an approved LPN school of nursing.
- One year clinical experience required
Licensure, Certifications, and Clearances:
CPR required based on AHA standards that include both a didactic and skills demonstration component within 30 days of hire
- Advanced Cardiac Life Support (ACLS) OR Basic Life Support (BLS) OR Cardiopulmonary Resuscitation (CPR)
- Licensed Practical Nurse (LPN)
- Act 33 with renewal
- Act 34 with renewal
- Act 73 FBI Clearance with renewal
