Description
The UPMC Harrisburg Renal Transplant team is in need of a Registered Nurse (RN) to join their growing and diverse team. The Clinical Transplant Coordinator (RN) will join a supportive team atmosphere where they build a long-term relationship with patients after their transplant surgery.
Hours: Monday - Friday 7:30am-4pm
Responsibilities:
- Clinical Transplant Coordinator I will provide care to post transplant patients.
- Provide teaching, evaluation in clinics, medication adjustments per established protocols, and longitudinal care/management after transplant.
- Teaching patients about post transplant care prior to discharge
- Preparing for clinic visits with patients, seeing patients in clinic and clinic follow-up (includes med review, reinforce teaching, completing nursing assessment, order/scheduling any follow-up testing etc.)
- Answering phone calls from patients
- Reviewing patient lab results and making adjustments based on established protocols
TRANSPLANT/VAD REFERRAL AND EVALUATION
- Identifies appropriate transplant/VAD candidates upon referral and interacts with appropriate hospital staff.
- Identifies requirements for candidate evaluation.
- Coordinates the evaluation process for potential transplant/VAD candidates with the multidisciplinary team.
- Functions as candidate advocate by applying knowledge of growth, development, educational and cultural background during candidate and family interaction.
- Provides data to regulatory agencies.
- Demonstrates knowledge of compliance with United Network for Organ Sharing (UNOS) policies and listing requirements.
Qualifications
- 2 years recent clinical nursing experience
- Demonstrated competence in professional nursing practice.
- Diploma or Associates Degree in Nursing required
- BSN preferred
- ICU, critical care or other nursing experience that enabled critical thinking and decision-making skills.
Licensure, Certifications, and Clearances:
- Current Pennsylvania licensure as a Registered Professional Nurse.
Clearances must be dated within 90 days
- Registered Nurse (RN)
- Act 33 with renewal
- Act 34 with renewal
- Act 73 FBI Clearance with renewal
