Description

UPMC has an exciting opportunity for a Care Manager based in State College, PA. This is a regular, full-time position within Community Care Behavioral Health (CCBH) with hybrid work arrangments (other than caseload dependent travel and in-person meetings, all work can be done remotely). 

A Care Manager is responsible for assisting members identified at risk for recidivism, discontinuous care, or as members of priority orspecial needs populations at all levels of the continuum. The Care Manager also manages care coordination for the member as needed. (Care Manager responsibilities are conducted under the supervision of a licensed professional.) The Care Manager executes these responsibilities consistent with the applicable Community Care Policies andProcedures. A care manager represents the organization to providers, member groups and families, and participates in the overalladministration of clinical operations as warranted.

Responsibilities:

  • Collaborates with community based case management services as appropriate.
  • Facilitates linkages for members and families between primary care and behavioral health providers and other social service or provider agencies as needed to develop and coordinate service plans.
  • Utilizes supervision with medical director and clinical manager regularly.
  • Participates in professional development activities.
  • Responds to deadlines and has work completed on or before deadline 95% of the time.
  • Proficiency in operating computer software
  • Works with members and providers to customize services to best meet members' needs within the scope of Community Care's obligations to its members.
  • Demonstrates excellent written and oral communication skills.
  • Demonstrates knowledge of clinical treatment, case management and community resources.
  • Participates in CQI activities and provider training.
  • Provides members, providers, and other stakeholders with accurate information concerning behavioral health care benefits and coverage.
  • Participates in the collection of information regarding the delivery and outcomes of services to members
  • Maintains close working relationships with the clinical leadership team
  • Maintains an understanding of behavioral health benefits and remains current on covered benefits, limitations, exclusions, and policies and procedures, in regards to services.
  • Assists members with smooth transition when moving into or out of the county.
  • Encourages coordination of care with primary care physician and other service providers integral to the member's life.
  • Works with Member Services, Network Management and Quality Management staff to assure that systematic revisions to improve services are developed and implemented.
  • Responds to member and provider complaints according to Community Care's policies and procedures.
  • Identifies provider issues and recommendations for improvement.
  • May engage in the acquisition or transfer of structured clinical data, but does not engage in any activities that involve clinical evaluation or interpretation.
  • Assists with coordinating information and making presentations to participating providers, state and federal agencies, community groups and other interested parties.

 

Qualifications

  • Master's degree in health or human services field
  • Minimum of three years of relevant clinical experience
  • Experience in managed care strongly preferred.
  • Experience in a child focused setting strongly preferred.
  • Experience in behavioral health strongly preferred.
  • Certification in substance use disorders helpful.
  • General knowledge of best practices in behavioral health, emphasizing work with special needs or priority populations and in public sector systems.

Licensure, Certifications, and Clearances:

  • Act 33 with renewal
  • Act 34 with renewal
  • Act 73 FBI Clearance with renewal
