UPMC complies with all governmental requirements related to local, state, and federal COVID-19 vaccination for employment. The Jan. 13 Supreme Court of the United States decision that the Centers for Medicare & Medicaid Services federal COVID-19 vaccine mandate will move forward requires UPMC to ensure employees either get vaccinated or receive a requested medical or religious exemption.

Summary:
UPMC is hiring full-time RN for the Home Health Team working Monday - Friday daylight. As the resource nurse, you will travel throughout multiple teams and areas depending on where the patient's needs are for the team. There will be occasional weekend and holiday rotations.   If you want to make a difference with direct patient care, this is the perfect role for your career!

Responsibilities:

  • Provides direct patient care, evaluates outcomes, and adjusts nursing care process as indicated to ensure optimal patient care. Initiates, coordinates, and evaluates patient and family centered health teaching.
  • Completes documentation including visit assessment, OASIS, 485 and Verbal Orders accurately and promptly on the laptop device or in written documentation while in the patient's home and within the agencies and regulatory standards. Data syncs (transmits) information on the same day.
  • Coordinates case management activities within a multi-disciplinary health care team to insure appropriate delivery of services to patients.
    Initiates, coordinates, and insures ongoing communication with the physician and other members of the health care team.

 

Qualifications

  • Graduate of an accredited school of professional nursing - Required
  • Six (6) months experience in Acute Care, Skilled Care Facility, Home Health, or Hospice Nursing required.
  • BSN - Preferred UPMC Visiting Nurses Association approved certification - Preferred 
  • One (1) year of home health experience - Preferred Annual performance rating of good, solid, strong performance or higher on most recent EPR The individual must be able to demonstrate the knowledge and skills necessary to provide care and/or interact appropriately to the ages of the patients served by his/her assigned unit as specified below.  Mobility and visual manual dexterity. Physical stamina for frequent walking, standing, lifting, and positioning of patients.


Licensure, Certifications, and Clearances:

  • Automotive Insurance
  • Basic Life Support (BLS) OR Cardiopulmonary Resuscitation (CPR)
  • Driver's License
  • Registered Nurse (RN)
  • Act 33
